慢性阻塞性肺疾病急性加重期降钙素原、趋化因子、N-末端脑钠肽前体与心肌损害的关系 被引量:4

The relationship between the procalcitonin,chemokines and N-terminal pro-BNP and the myocardial damage in acute exacerbation of chronic obstructive pulmonary disease

摘要 目的 分析慢性阻塞性肺疾病急性加重(AECOPD)患者血清降钙素原(PCT)、单核细胞趋化蛋白-1(MCP-1)/CC类趋化因子配体2(CCL2)及N-末端脑钠肽前体(NT-proBNP)的变化情况,探讨其水平与心肌损伤程度的关系。方法 回顾性分析2016年1月至2021年1月中国人民解放军西部战区总医院和四川省人民医院收治的120例COPD患者临床资料,其中AECOPD患者36例为观察组,COPD稳定期患者84例为对照组,比较两组血清PCT、MCP-1/CCL2及NT-proBNP水平,分析其与患者病情严重程度及心肌损伤程度指标肌钙蛋白I(cTnI)的关系。结果 两组血清cTnI水平、急性生理与慢性健康II(APACHE II)评分、血清PCT、MCP-1/CCL2和NT-proBNP水平差异有统计学意义(P<0.05);COPD患者病情严重程度与血清PCT、MCP-1/CCL2和NT-proBNP水平呈正相关(P<0.05);AECOPD患者血清cTnI水平与PCT、MCP-1/CCL2和NT-proBNP水平呈正相关(P<0.05)。结论 随COPD患者病情恶化,其血清PCT、MCP-1/CCL2及NT-proBNP表达水平逐渐升高,且各指标水平升高可能表明AECOPD患者心肌损伤程度加剧。 Objective To analyze the changes of serum procalcitonin(PCT), monocyte chemoattractant protein-1(MCP-1)/C-C motif chemokine ligand 2(CCL2) and N-terminal pro-B-type natriuretic peptide(NT-proBNP) in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD), and explore the relationship between these indicators and the degree of myocardial damage.Methods Clinical data of 120 patients with COPD admitted to our hospitals from January 2016 to January 2021 were retrospectively analyzed. There were 36 patients with AECOPD(observation group) and 84 patients with stable COPD(control group). The levels of serum PCT, MCP-1/CCL2 and NT-proBNP were compared between the two groups. The correlation of the severity of COPD with cardiac troponin I(cTnI), a myocardial damage indicator, was analyzed.Results There were statistically significant differences in terms of serum cTnI level, Acute Physiology and Chronic Health Evaluation II(APACHE II) scores and levels of serum PCT, MCP-1/CCL2 and NT-proBNP(P<0.05) between the two groups. The severity of COPD was positively correlated with levels of serum PCT, MCP-1/CCL2 and NT-proBNP(P<0.05). The cTnI level was positively correlated with the levels of serum PCT, MCP-1/CCL2 and NT-proBNP(P<0.05).Conclusions With the deterioration of COPD, levels of serum PCT, MCP-1/CCL2 and NT-proBNP are increased. The increased levels may indicate the aggravation of myocardial damage in patients with AECOPD.
