摘要
目的应用迷你临床演练评量(mini-clinical evaluation exercise,Mini-CEX)评价临床路径(clinical pathway,CP)联合以问题为导向的教学法(problem-based learning,PBL)在内分泌科住院医生规范化培训中的效果。方法将2019年1月—2020年6月在南京医科大学附属淮安第一医院内分泌科接受住培的60名住院医生分为CP组、PBL组和试验组(采用CP联合PBL教学法),三组在培训前后均采用Mini-CEX进行评价,并对带教老师和学员的满意度进行调查。结果培训结束后试验组住培医师在医疗面谈、临床判断、组织效能及整体表现方面均显著优于CP组和PBL组,体格检查方面显著优于CP组,差异有统计学意义(P<0.05)。教师对学员满意度及学员对教师满意度,均是培训后显著高于培训前,且试验组显著高于另两组,差异有统计学意义(P<0.05)。结论CP联合PBL教学法能够更好得提高规范化培训住院医师的临床实践及解决问题的能力。用Mini-CEX可以更直观地评价不同教学方法的培训效果。
Objective To evaluate the efficacy of CP combined with PBL teaching method in standardized training of resident doctors in endocrinology department by Mini-CEX.Methods From January 2019 to June 2020,60 residents who received standardized training in the department of endocrinology of the Affiliated Huai’an No.1People’s Hospital of Nanjing Medical University,were divided into CP group,PBL group and experimental group(using CP combined with PBL teaching method).The three groups were evaluated by Mini-CEX before and after the training,and the satisfaction of teachers and students was investigated.Results After the training,the residents in the test group were significantly better than those in the CP group and PBL group in terms of medical interview,clinical judgment,organizational efficiency and overall performance,and the physical examination was significantly better than that in the CP group,with a statistically significant difference(P<0.05).The teachers’satisfaction with students and the students’satisfaction with teachers in group A were significantly higher after training than before,and the test group was significantly higher than the other two groups,with a statistically significant difference(P<0.05).Conclusion CP combined with PBL teaching method can better improve the clinical practice and problem-solving ability of standardized training residents.Mini-CEX can be used to evaluate the training effect of different teaching methods more intuitively.
