摘要
目的探讨慢性阻塞性肺疾病患者血清25羟基维生素D[25-(OH)D]与肺功能的相关性。方法选取2021年10月至2022年4月江西省景德镇市第五人民医院呼吸科收治的115例COPD患者作研究对象,根据病情分期分为慢性阻塞性肺疾病急性加重期(AECOPD)组(40例)、COPD稳定组(75例),并选取同期年龄、性别分布相似的65例健康志愿者为对照组,比较三组的血清25-(OH)D水平及肺功能[1秒用力呼气容积(FEV1)/用力肺活量(FVC)],通过慢性阻塞性肺病疾病评分(CAT)将COPD患者分层,探讨其与血清25-(OH)D水平的相关性。结果AECOPD组、COPD稳定组的血清25-(OH)D及FEV1/FVC均低于对照组,且AECOPD组低于COPD稳定组,差异有统计学意义(P<0.05);Pearson相关系数分析显示,血清25-(OH)D水平与CAT得分呈负相关(P<0.05)。结论COPD患者血清25-(OH)D低于常人,25-(OH)D水平与COPD患者病情程度呈负相关,其随COPD病情进展呈下降趋势。
Objective To explore the correlation between serum 25-hydroxyvitamin D(25-[OH]D)and pulmonary function in patients with chronic obstructive pulmonary disease(COPD).Methods A total of 115 patients with COPD admitted to Department of Respiratory Medicine of the Fifth People's Hospital of Jingdezhen City,Jiangxi Province were selected as research subjects from October 2021 to April 2022,and they were divided into acute exacerbation of chronic obstructive pulmonary disease(AECOPD)group(40 cases)and stable COPD group(75 cases)according to disease staging,and 65 healthy volunteers with similar age and gender during the same period were recruited as control group.Serum 25-(OH)D level and pulmonary function(forced expiratory volume in 1 second[FEV1]/forced vital capacity[FVC])were compared among the three groups.The patients with COPD were stratified by COPD assessment test(CAT),and the correlation with serum 25-(OH)D level was explored.Results Serum 25-(OH)D and FEV1/FVC in AECOPD group and stable COPD group were lower than those in control group,and the two indicators in AECOPD group were lower than those in stable COPD group,with statistically significant differences(P<0.05).Pearson correlation coefficient analysis showed that serum 25-(OH)D level was negatively correlated with CAT score(P<0.05).Conclusion Serum 25-(OH)D in patients with COPD is lower than that in normal people.The level of 25-(OH)D is negatively correlated with the disease severity of patients with COPD,and it tends to decrease with the progression of COPD.
