摘要
目的 构建鼻咽癌容积旋转调强放疗(VMAT)腮腺剂量预测模型,并验证其准确性。方法 选取自贡市第一人民医院45例和自贡市第四人民医院30例行VMAT的鼻咽癌患者,提取靶区和危及器官腮腺的解剖空间位置关系,以靶区体积、处方剂量、腮腺体积、靶区和腮腺空间位置量为输入量,以腮腺15%、30%、45%体积所占的剂量(D_(15)、D_(30)、D_(45))和平均剂量(D_(mean))作为输出量,经主成分分析建立回归方程模型预测腮腺剂量,并验证模型的准确性。结果 建立了腮腺剂量的预测模型方程。预测值与实际值相比,腮腺D15的相对误差在0.11%~3.13%,D30的相对误差在0.10%~3.72%,D45的相对误差在4.54%~10.53%,D_(mean)的相对误差在0.35%~5.56%。腮腺D_(30)实际值与预测值差异有统计学意义(P<0.05),D_(15)、D_(45)和D_(mean)差异均无统计学意义(P>0.05)。结论 该模型可以较为准确地预测鼻咽癌VMAT计划腮腺剂量。
Objective To construct the dose prediction model of volumetric modulated arc radiotherapy(VMAT) of parotid gland for nasopharyngeal carcinoma, and to verify its accuracy.Methods Forty-five cases of nasopharyngeal carcinoma undergoing VMAT in the Zigong Municipal First People’s Hospital and 30 cases of nasopharyngeal carcinoma undergoing VMAT in the Zigong Municipal Fourth People’s Hospital were selected.The anatomical spatial relationship of target areas and parotid gland of the organ at risk was extracted, and the target area volume, prescription dose, parotid gland volume, target area and spatial location amount of parotid gland were used as the inputs amounts, the dose occupied by 15%,30% and 45% of parotid gland volume(D_(15),D_(30),D_(45)) and mean dose(D_(mean)) served as the output amounts, the regression equation model for predicting the parotid gland dose was established by principal component analysis, and its accuracy was verified.Results The predictive model of parotid gland dose was established.In the comparison between the predictive value and actual measurement value, the relative error range of parotid D15was 0.11%-3.13%,which of D30was 0.10%-3.72%,which of D45was 4.54%-10.53%,and which of D mean was 0.35%-5.56%;the difference between the actual value of parotid gland D30and the predictive value was statistically significant(P<0.05),and which among D_(15),D_(45) and D_(mean) had no statistical difference(P>0.05).Conclusion The model could more accurately predict the dose of parotid gland in IMRT plan for nasopharyngeal carcinoma.
