基于精准医学理论指导下的早期分级肺康复训练在呼吸衰竭患者中的应用效果 被引量:9

Application Effects of Early Phased Pulmonary Rehabilitation Training on the Patients Suffering Respiratory Failure Under the Guidance of Precision Medicine Theory

摘要 目的:探讨基于精准医学理论指导下的早期分级肺康复训练在重症监护室呼吸衰竭患者中的应用效果。方法:将93例呼吸衰竭患者采用随机数字表法分为观察组47例、对照组46例。对照组给予常规肺康复训练,观察组给予基于精准医学理论指导下的早期分级肺康复训练,共干预7周。比较两组血气指标、氧合指数、急性生理和慢性健康评分、心肺功能、治疗及住院时间、呼吸机相关性并发症。结果:观察组动脉血氧分压、氧合指数高于对照组,动脉血二氧化碳分压、急性生理和慢性健康评分低于对照组(P<0.05)。心率低于对照组,用力呼气肺活量、第一秒用力呼气容积高于对照组(P<0.05)。机械通气时间、ICU治疗时间、住院时间短于对照组(P<0.05)。呼吸机相关性并发症发生率观察组[6.38%(3/47)]低于对照组[23.91%(11/46)](P<0.05)。结论:基于精准医学理论指导的早期肺康复训练有助于改善呼吸衰竭患者血气指标及心肺功能,缩短治疗时间,减少呼吸机相关性并发症的发生。 Objective:To explore the application effects of early phased pulmonary rehabilitation training on the patients suffering respiratory failure in ICU under the guidance of precision medicine theory.Methods:All 93patients with respiratory failure were allocated to 47 cases of the observation group and 46 cases of the control group by using random number table method.The control group accepted conventional lung rehabilitation training,and the observation group early phased pulmonary rehabilitation training under the guidance of precision medicine theory,intervention for seven weeks.To compare the indexes of blood gas,oxygenation index,acute physiological and chronic health scores,cardio-pulmonary function,therapeutic and hospitalization time,respirator-related complications between both groups.Results:The observation group was higher than the control group in arterial partial pressure of oxygen and oxygenation index,lower than the control group in partial pressure of carbon dioxide in artery,acute physiological and chronic health scores(P<0.05).The observation group was lower than the control group in heart rate,higher than the control group in forced vital capacity(FVC),forced expiratory volume within one second(FEV1)(P<0.05).The duration of mechanical ventilation,ICU treatment time and hospitalization time of the observation group were shorter than these of the control group(P<0.05).The incidence of respirator-related complications of the observation group was 6.38%(3/47),lower than 23.91%(11/46)of the control group(P<0.05).Conclusion:Early phased pulmonary rehabilitation training under the guidance of precision medicine theory could help improve the indexes of blood gas and cardio-pulmonary function in the patients suffering respiratory failure,shorten therapeutic time and reduce the incidence of respirator-related complications.
