摘要
目的探讨显微镜精索静脉结扎术(MSV)在日间病房结合快速康复外科(FTS)理念的临床应用效果。方法选取2017年3月至2020年3月青海红十字医院行MSV治疗的60例精索静脉曲张(VC)患者为研究对象,按随机数字表法将研究对象分为观察组和对照组,每组各30例。对照组行常规MSV治疗,观察组在对照组治疗方案的基础上行日间病房并结合应用FTS理念。对两组患者的住院时间、总输液量、住院总费用、术后并发症发生率、精液质量改善情况进行对比分析。结果观察组患者的住院时间、总输液量、住院总费用均显著低于对照组,差异均有统计学意义(均P<0.05)。两组患者术后3个月的精子浓度、精子活动率、精子活力及术前精液分析检查较术前明显改善,差异均有统计学意义(均P<0.05)。结论MSV日间病房结合FTS理念安全可靠,大幅度缩减了患者住院时间、输液量及住院总费用,未增加术后睾丸萎缩、睾丸鞘膜积液、伤口感染的发生率,不影响精液质量改善率,值得临床推广及开展。
Objective The clinical application effect of microscope spermatic vein(MSV)ligation in the day ward combined with the concept of fast track surgy(FTS).Methods A total of 60 MSV inpatients in our hospital from March 2017 to March 2020 were selected as the research objects.According to the random number table method,the research objects were divided into observation group(n=30)and control group(n=30).The observation group was ascended to the day ward on the basis of the treatment plan of the control group and combined with the application of the FTS concept.The two groups of patients were compared and analyzed the hospitalization time,total infusion volume,total hospitalization cost,postoperative complication rate,and semen quality improvement.Results The hospitalization time,total infusion volume,and total hospitalization expenses of the observation group were significantly lower than those of the control group,and the differences were statistically significant(all P<0.05).There was no significant difference in postoperative complications and semen quality improvement between the two groups of patients(P>0.05).Three months after surgery,the sperm concentration,sperm motility,sperm motility and preoperative semen analysis of the two groups were significantly improved compared with those before surgery,and the differences were statistically significant(all P<0.05).Conclusions The MSV day ward combined with the FTS concept is safe and reliable,which greatly reduces the patients′hospitalization time,infusion volume and hospitalization costs,does not increase the incidence of postoperative testicular atrophy,testicular hydrocele,and wound infection,and does not affect the improvement rate of semen quality.It is worthy of clinical promotion and development.
