骨代谢生化指标预测绝经后女性骨质疏松性椎体骨折的价值

摘要 目的探讨骨代谢生化指标预测绝经后女性骨质疏松性椎体骨折的价值。方法选取中山市博爱医院2019年1月至2022年4月80例绝经后骨质疏松性椎体骨折的门诊及住院患者作为观察组,并选取同期80例无骨折的健康绝经后妇女作为对照组。两组均行骨代谢生化指标检验,比较两组检验结果,并分析骨代谢生化指标与绝经后女性骨质疏松性椎体骨折的相关性。结果两组在甲状旁腺激素(PTH)、血钙、血磷值、骨钙素、骨碱性磷酸酶方面比较,差异无统计学意义(P>0.05)。观察组的Ⅰ型前胶原氨基端前肽(PINP)、Ⅰ型胶原羧基端肽β特殊序列(β-CTX)水平高于对照组(P<0.05),而25-羟基维生素D水平低于对照组(P<0.05)。相关性分析显示,PINP、骨钙素、骨碱性磷酸酶、β-CTX与绝经后女性骨质疏松性椎体骨折呈正相关性(P<0.05),而25-羟维生素D水平与绝经后女性骨质疏松性椎体骨折呈负相关性(P<0.05)。结论骨代谢生化指标能预测绝经女性椎体骨折的发生风险,有利于尽早干预,以降低骨折发生率,临床可推广使用。 Objective To investigate the value of bone metabolism biochemical indexes in predicting osteoporotic vertebral fractures in postmenopausal women.Methods A total of 80 postmenopausal outpatients and inpatients with osteoporotic vertebral fractures in Boai Hospital of Zhongshan from January 2019 to April 2022 were selected as the observation group,and 80 healthy postmenopausal women without fractures were selected as the control group during the same period.The biochemical indexes of bone metabolism were tested in both groups.The test results of the two groups were compared,and the correlation between the biochemical indexes of bone metabolism and osteoporotic vertebral fractures in postmenopausal women was analyzed.Results There were no significant differences in parathyroid hormone(PTH),serum calcium,serum phosphorus,osteocalcin and bone alkaline phosphatase between the two groups(P>0.05).Propeptide of typeⅠprocollagen(PINP)andβ-C-terminal telopeptide of typeⅠcollagen(β-CTX)in the observation group were higher than those in the control group(P<0.05).But the level of 25-hydroxy vitamin D in the observation group was lower than that in the control group(P<0.05).The correlation analysis showed that PINP,osteocalcin,bone alkaline phosphatase andβ-CTX were positively correlated with osteoporotic vertebral fractures in postmenopausal women(P<0.05),while level of 25-hydroxy vitamin D was negatively correlated with osteoporotic vertebral fractures in postmenopausal women(P<0.05).Conclusion Biochemical indexes of bone metabolism can predict the risk of vertebral fracture in postmenopausal women,which is conducive to early intervention to reduce the incidence of fracture,and can be widely used in clinic.
