摘要
目的探讨超声检查在不同类型小儿阑尾炎临床筛查中的应用价值。方法选取我院2019年1月至2021年4月收治的300例经病理检查确诊的阑尾炎患儿,所有患儿均行超声检查,根据病理检查结果分析超声检查对不同类型小儿阑尾炎的诊断价值及影像学特征。结果超声检查诊断单纯性阑尾炎、化脓性阑尾炎及坏疽性阑尾炎的正确率分别为97.74%、96.81%、93.15%。超声检查诊断单纯性阑尾炎、化脓性阑尾炎的正确率与病理检查比较无统计学差异(P>0.05),诊断坏疽性阑尾炎的正确率低于病理检查(P<0.05)。超声检查显示,单纯性阑尾炎、化脓性阑尾炎及坏疽性阑尾炎的阑尾长度比较无明显差异(P>0.05),但宽度及管壁厚度比较均存在明显差异(P<0.05)。结论超声检查对不同类型小儿阑尾炎的鉴别诊断准确度较高,可作为诊断不同类型阑尾炎的首选方式。
Objective To explore the application value of ultrasonography in clinical screening of different types of appendicitis in children.Methods 300 children with appendicitis diagnosed by pathological examination in our hospital from January 2019 to April 2021 were selected and given ultrasonography.According to the pathological examination results,the diagnostic value and imaging characteristics of ultrasonography in different types of appendicitis in children were analyzed.Results The accuracy of ultrasonography in the diagnosis of simple appendicitis,suppurative appendicitis and gangrenous appendicitis were 97.74%,96.81%and 93.15%respectively.No statistical difference was found in the accuracy of ultrasonography in the diagnosis of simple appendicitis and suppurative appendicitis between ultrasonography and pathological examination(P>0.05);The accuracy of ultrasonography in the diagnosis of gangrenous appendicitis was lower than that of pathological examination(P<0.05).Ultrasonography showed no significant difference was found in the appendix length among simple appendicitis,suppurative appendicitis and gangrenous appendicitis(P>0.05),but showed significant difference in the width and wall thickness(P<0.05).Conclusions Ultrasonography has higher accuracy in the differential diagnosis of different types of appendicitis in children,and can be used as first choice for the diagnosis of different types of appendicitis.
