摘要
目的探讨肺癌组织中CYP1B1的表达与临床病理特征和预后之间的关系。方法135名NSCLC患者的肿瘤组织和匹配的癌旁正常组织样本。对NSCLC患者随访5年。采用免疫组织化学法分析组织中CYP1B1的表达。Kaplan-Meier法和Cox风险回归模型用于确定CYP1B1与NSCLC患者预后的关系。结果与癌旁正常组织相比,肺癌组织中CYP1B1的表达显著增加(t=18.176,P<0.05)。此外,肿瘤大小≥4 cm、TNM分期Ⅲ+Ⅳ、淋巴结转移患者肺癌组织中CYP1B1高表达的比例明显高于肿瘤大小<4 cm(P=0.009)、TNM分期Ⅰ+Ⅱ(P=0.031)、无淋巴结转移(P=0.047)的患者(P<0.05)。Kaplan-Meier法显示,CYP1B1高表达患者的总生存率明显低于CYP1B1低表达患者(log-rankχ^(2)=7.525,P=0.006)。Cox回归分析显示,肿瘤分化(P=0.013)、淋巴结转移(P=0.007)和CYP1B1表达(P=0.031)是影响肺癌患者预后的独立危险因素。结论肺癌组织中CYP1B1表达的升高与肿瘤大小、TNM分期和淋巴结转移显著相关,是影响肺癌预后的独立影响因素。CYP1B1表达升高可作为NSCLC患者预后不良的新型预测因子。
Objective To analyze the relationship between CYP1B1 expression and clinical parameters in NSCLC patients.Methods Tumor tissue and matched adjacent normal tissue samples from 135 NSCLC patients were collected.NSCLC patients were followed up for 5 years.The expression of CYP1B1 in tissues was analyzed by immunohistochemistry.Kaplan-Meier method was used to determine the relationship between CYP1B1 and overall survival in NSCLC patients,and Cox regression model was used to analyze factors associated with the prognosis of NSCLC patients.Results The expression of CYP1B1 in tumor tissues was markedly higher than that of adjacent normal tissues(t=18.176,P<0.05).The proportion of high expression of CYP1B1 in lung cancer tissues of patients with tumor size≥4 cm,TNM stage Ⅲ+Ⅳ,and lymph node metastasis was significantly higher than that of patients with tumor size<4 cm,TNM stage Ⅰ+Ⅱ,and no lymph node metastasis.The Kaplan-Meier revealed that individuals with higher CYP1B1 levels had a considerably worse overall survival rate than individuals with lower CYP1B1 levels(log-rankχ^(2)=7.525,P=0.006).Cox regression analysis showed that tumor differentiation(P=0.013),lymph node metastasis(P=0.007)and CYP1B1 expression(P=0.031)were independent risk factors for the prognosis of lung cancer patients.Conclusion The increased expression of CYP1B1 in lung cancer tissue was significantly correlated with tumor size,TNM stage and lymph node metastasis,and was an independent factor affecting the prognosis of lung cancer.Elevated expression of CYP1B1 may serve as a novel predictor of poor prognosis in NSCLC patients.
