摘要
目的探讨针灸结合通痹益脑汤治疗脑梗死后血管性痴呆患者的临床效果。方法选取2019年1月至2020年12月我院收治的80例脑梗死后血管性痴呆患者作为研究对象,按随机数字表法将其分为对照组与观察组,每组40例。对照组采用通痹益脑汤治疗,观察组采用针灸结合通痹益脑汤治疗。比较两组的脑血流灌注情况、智力状态与生活能力评分、血清生化指标以及临床疗效。结果治疗后,观察组的大脑中动脉最大血流速度(Vmax)、最小血流速度(Vmin)、平均血流量(Qmean)均显著高于对照组,阻力指数(RI)显著低于对照组,差异具有统计学意义(P<0.05)。治疗后,观察组的智能精神状态检查量表(MMSE)、日常生活能力评定量表(ADL)评分均高于对照组,差异具有统计学意义(P<0.05)。治疗后,观察组的神经元特异性烯醇化酶(NSE)水平显著低于对照组,神经生长因子(NGF)、神经营养因子(NTF)水平均显著高于对照组,差异具有统计学意义(P<0.05)。观察组的治疗总有效率为97.50%,高于对照组的80.00%,差异具有统计学意义(P<0.05)。结论针灸结合通痹益脑汤治疗脑梗死后血管性痴呆患者能取得较好的效果,不仅能改善脑血流灌注情况,还能提高患者的智力水平与自理能力,对加快患者康复具有非常积极的意义,值得临床推广及应用。
Objective To explore the clinical effect of acupuncture combined with Tongbi Yinao decoction in the treatment of patients with vascular dementia after cerebral infarction.Methods A total of 80 patients with vascular dementia after cerebral infarction admitted in our hospital from January 2019 to December 2020 were selected as the research objects and divided into control group and observation group according to the random number table method,with 40 cases in each group.The control group was treated with Tongbi Yinao decoction,and the observation group was treated with acupuncture combined with Tongbi Yinao decoction.The cerebral blood flow perfusion,intelligence status and living ability scores,serum biochemical indexes and clinical efficacy were compared between the two groups.Results After treatment,the maximum blood flow velocity(Vmax),minimum blood flow velocity(Vmin)and mean blood flow(Qmean)of the middle cerebral artery in the observation group were significantly higher than those in the control group,the resistance index(RI)was significantly lower than that in the control group,and the differences were statistically significant(P<0.05).After treatment,the scores of Mini-Mental State Examination(MMSE),Activity of Daily Living(ADL)in the observation group were significantly higher than those in the control group,and the differences were statistically significant(P<0.05).After treatment,the level of neuron specific enolase(NSE)in the observation group was significantly lower than that in the control group,the levels of nerve growth factor(NGF),neurotrophic factor(NTF)were significantly higher than those in the control group,and the differences were statistically significant(P<0.05).The total effective rate of treatment of the observation group was 97.50%,which was higher than 80.00%of the control group,and the difference was statistically significant(P<0.05).Conclusion Acupuncture combined with Tongbi Yinao decoction in the treatment of patients with vascular dementia after cerebral infarction has a good effect,it can not only improve the cerebral blood flow perfusion,but also improve the intellectual level and self-care ability of patients.It has a very positive significance to accelerate the rehabilitation of patients,and is worthy of clinical promotion and application.
