摘要
目的 探讨经病理诊断乳腺肿瘤患者时采取免疫组化技术提高准确率的价值。方法 回顾性选取2018年7月—2021年6月湛江中心人民医院高危乳腺肿瘤患者164例,均接受病理诊断,分析免疫组化技术检测结果、免疫组化技术检测价值、免疫组化检测指标与乳腺癌的相关性。结果 164例患者中,病理检查阳性66例,阴性98例;免疫组化技术检测ER阳性56例、阴性108例,AR阳性88例、阴性76例,HER-2阳性88例、阴性76例,CK5/6阳性98例、阴性66例,CK34βE12阳性88例、阴性76例,P53阳性84例、阴性80例,P63阳性84例、阴性80例,Ki67阳性90例、阴性74例;与病理检查对比,免疫组化技术检测AR、HER-2、CK5/6的灵敏性、特异性、准确性、阳性预测值、阴性预测值均高于CK34βE12、P53、P63、Ki67,差异有统计学意义(P<0.05)。ER的灵敏性、阴性预测值均低于AR、HER-2、CK5/6、CK34βE12、P53、P63、Ki67,差异有统计学意义(P<0.05);特异性、准确性、阳性预测值均高于AR、HER-2、CK5/6、CK34βE12、P53、P63、Ki67,差异有统计学意义(P<0.05)。免疫组化检测ER与乳腺癌呈显著的负相关关系(P<0.05);AR、HER-2、CK5/6、CK34βE12、P53、P63、Ki67与乳腺癌均呈显著的正相关关系(P<0.05)。结论 经病理诊断乳腺肿瘤患者时采取免疫组化技术能够有效提高准确率。
Objective To explore the value of taking immunohistochemistry techniques to improve the accuracy rate when diagnosing breast tumor patients by pathology. Methods A total of 164 patients with high-risk breast tumors in Zhanjiang Central People’s Hospital from July 2018 to June 2021 were retrospectively selected. All received pathological diagnosis, and the results of immunohistochemical techniques, the value of immunohistochemical techniques for detection, and the correlation between immunohistochemical detection indexes and breast cancer were analyzed. Results Among 164 patients, 66 were pathologically positive and 98 were negative;Immunohistochemical techniques detected 56 positive cases of ER and 108 negative cases, 88 positive cases of AR and 76 negative cases, 88 positive cases of HER-2 and 76 negative cases, 98 positive cases of CK5/6 and 66 negative cases, 88 positive cases of CK34βE12 and 76 negative cases, 84 positive cases of P53 and 80 negative cases, 84 positive cases of P63 and 80 negative cases, 90positive cases of Ki67 and 74 negative cases;compared with pathological examination, the sensitivity, specificity, accuracy, positive predictive value, and negative predictive value of immunohistochemical technique to detect AR,HER-2, and CK5/6 were higher than those of CK34βE12, P53, P63, and Ki67, the difference was statistically significant(P<0.05). and the sensitivity and negative predictive value of ER were lower than those of AR, HER-2, CK5/6, CK34βE12, P53 P63, and Ki67, the difference was statistically significant(P<0.05);the specificity, accuracy, and positive predictive value were higher than AR, HER-2, CK5/6, CK34βE12, P53, P63, and Ki67, the difference was statistically significant(P<0.05). Immunohistochemical detection of ER showed a significant negative correlation with breast cancer(P<0.05);and AR, HER-2, CK5/6, CK34βE12, P53, P63, and Ki67 showed a significant positive correlation with breast cancer(P<0.05). Conclusion Immunohistochemical techniques can effectively improve the accuracy rate when diagnosing breast tumor patients by pathology.
