定量CT在2型糖尿病患者肝脏脂肪含量定量分析中的应用价值 被引量:2

Application value of quantitative CT in the quantitative analysis of liver fat content for patients with type 2 diabetes mellitus

摘要 目的探讨定量CT(QCT)在2型糖尿病患者肝脏脂肪含量定量分析中的应用价值。方法纳入70例2型糖尿病患者。所有患者均进行全腹部CT平扫和MRI检查,分别采用QCT系统和定量非对称回波的最小二乘估算法迭代水脂分离序列(IDEAL-IQ)技术检测肝脏脂肪含量。以IDEAL-IQ所测量的肝脏质子密度脂肪分数(PDFF)为金标准,采用Pearson检验分析QCT所测量的肝脏脂肪含量与肝脏PDFF的相关性,采用Bland-Altman法分析QCT所测量的肝脏脂肪含量与肝脏PDFF的一致性。结果70例2型糖尿病患者的脂肪肝发生率为48.57%(34/70),肝脏PDFF为(5.58±2.56)%,QCT所测量的肝脏脂肪含量为(9.61±3.43)%。Pearson检验结果显示,QCT所测量的肝脏脂肪含量与肝脏PDFF具有相关性,在女性患者中相关性更显著(均P<0.05)。Bland-Altman一致性分析结果显示,95%一致性界限为(0.11%,8.38%),95%一致性界限外的病例占比为2.8%,两种方法检测所得的肝脏脂肪含量具有较好的一致性。结论QCT所测量的肝脏脂肪含量与肝脏PDFF具有相关性和一致性,且在女性2型糖尿病患者中相关性更强。两种方法均能精确定量分析2型糖尿病患者的肝脏脂肪含量,在临床应用中或许可以相互替换。 Objective To explore the application value of quantitative CT(QCT)in the quantitative analysis of liver fat content in patients with type 2 diabetes mellitus.Methods Seventy patients with type 2 diabetes mellitus were enrolled.All patients underwent total abdominal CT plain scan and MRI examination,and liver fat content was measured by QCT system and iterative decomposition of water and fat with echo asymmetry and least-squares estimation quantification sequence(IDEAL-IQ)technique,respectively.The liver proton density fat fraction(PDFF)measured by IDEAL-IQ was used as the gold standard,and the correlation between liver fat content measured by QCT and liver PDFF was analyzed by the Pearson test.The Bland-Altman method was used to analyze the consistency between liver fat content measured by QCT and liver PDFF.Results The incidence rate of fatty liver in 70 patients with type 2 diabetes mellitus was 48.57%(34/70),liver PDFF was(5.58±2.56)%,and liver fat content measured by QCT was(9.61±3.43)%.The results of Pearson test interpreted that liver fat content measured by QCT had correlation with liver PDFF,and this correlation was more obvious in female patients(all P<0.05).The results of Bland-Altman consistency analysis revealed that 95%consistency limits were(0.11%,8.38%),the proportion of cases outside the 95%consistency limit was 2.8%,and the liver fat content measured by the two methods exerted a relatively good consistency.Conclusion The liver fat content measured by QCT has correlation and consistency with liver PDFF,and the correlation is stronger in female patients with type 2 diabetes mellitus.The two methods can accurately quantify the liver fat content in patients with type 2 diabetes mellitus,and may be replaced by each other in clinical application.
