摘要
目的:探究呼吸道感染早产儿脂溶性维生素水平,研究其与免疫能力之间的关系。方法:前瞻性选取2020年5月—2022年5月在本院新生儿监护室收治的100例呼吸道感染早产儿为研究对象,另按1:1比例选取同期我院新生儿科正常早产儿100例为对照组,通过高效液相色谱法检测2组血清维生素A、维生素D、维生素E水平,流式细胞仪检测CD3^(+)、CD4^(+)、CD3^(+)/CD8^(+)、CD4^(+)/CD8^(+)、CD3^(+)/CD19^(+)、CD19^(+)/CD23^(+)水平,免疫比浊法检测IgG、IgM、IgA水平。比较2组早产儿脂溶性维生素水平、免疫功能指标差异,并使用Pearson相关性分析脂溶性维生素水平与免疫功能相关性。结果:研究组维生素A、维生素D、维生素E水平低于对照组,其缺乏率高于对照组(P<0.05)。另研究组CD3^(+)、CD4^(+)、CD3^(+)/CD8^(+)、CD4^(+)/CD8^(+)、IgG、IgM、IgA低于对照组(P<0.05),而CD3^(+)/CD19^(+)、CD19^(+)/CD23^(+)高于对照组(P<0.05)。Pearson相关性显示,维生素A、维生素D、维生素E与CD3^(+)、CD4^(+)、CD3^(+)/CD8^(+)、CD4^(+)/CD8^(+)、IgG、IgM、IgA呈正相关,与CD3^(+)/CD19^(+)、CD19^(+)/CD23^(+)呈负相关(P<0.05)。结论:呼吸道感染早产儿存在脂溶性维生素水平欠缺与免疫功能下降,且脂溶性维生素水平与免疫功能存在相关。
【Objective】 To investigate the level of fat-soluble vitamins in preterm infants with respiratory tract infection and to study the relationship between them and immunity.【Method】 premature infants with respiratory tract infection who were admitted to the neonatal intensive care unit of our hospital from May 2020 to May 2022 were prospectively selected as the research objects, and 100 normal premature infants in the neonatal department of our hospital during the same period were selected as the control at a ratio of 1:1. The serum levels of vitamin A,vitamin D,and vitamin E in the two groups were detected by high performance liquid chromatography, and the levels of CD3^(+),CD4^(+),CD3^(+)/CD8^(+),CD4^(+)/CD8^(+),CD3^(+)/CD19^(+),CD19^(+)/CD23^(+)were detected by flow cytometry, and the levels of IgG,IgM and IgA were detected by turbidimetry. The levels of fat-soluble vitamins and immune function indexes were compared between the two groups of premature infants, and Pearson correlation was used to analyze the relationship between fat-soluble vitamin levels and immune function.【Result】 The levels of vitamin A,vitamin D,and vitamin E in the study group were lower than those in the control group, and the deficiency rates of them were higher than those in the control group(P<0.05). In addition, CD3^(+),CD4^(+),CD3^(+)/CD8^(+),CD4^(+)/CD8^(+),IgG,IgM,and IgA in the study group were lower than those in the control group(P<0.05),while CD3^(+)/CD19^(+),CD19^(+)/CD23^(+)were higher than those in the control group(P<0.05). Pearson correlation showed that vitamin A,vitamin D,vitamin E were positively correlated with CD3^(+),CD4^(+),CD3^(+)/CD8^(+),CD4^(+)/CD8^(+),IgG,IgM,IgA,and negatively correlated with CD3^(+)/CD19^(+),CD19^(+)/CD23^(+)(P<0.05). 【Conclusion】Preterm infants with respiratory tract infection have deficiency of fat-soluble vitamin levels and decreased immune function, and there is a correlation between fat-soluble vitamin levels and immune function.
