摘要
目的:探究盆底超声测量前腔室结构参数对初产妇产后压力性尿失禁(SUI)的诊断价值。方法:选取2020年1月-2022年4月于赣州市人民医院分娩并于产后42 d回院复查的86例初产妇,所有产妇均进行盆底超声检测,根据是否发生SUI分为SUI组(37例)与非SUI组(49例)。比较两组初产妇最大Valsalva动作、静息时的前腔室结构参数,分析其对SUI的诊断价值。结果:最大Valsalva动作时,SUI组膀胱颈至参考线垂直距离(BNS)低于非SUI组,膀胱颈移动度(BND)、尿道旋转角(URA)均高于非SUI组(P<0.05)。静息时,SUI组膀胱颈位置(BNP)低于非SUI组,膀胱尿道后角(PUA)高于非SUI组(P<0.05)。绘制受试者工作特征(ROC)曲线,结果显示,BNS、BND、URA、BNP、PUA及联合检测诊断初产妇SUI的曲线下面积(AUC)分别为0.870、0.871、0.837、0.804、0.752、0.927,有一定诊断价值。结论:盆底超声测量前腔室结构参数可及早发现初产妇的盆底组织结构异常,对初产妇SUI具有较高诊断价值。
Objective: To investigate the diagnostic value of anterior chamber structure parameters measured by pelvic floor ultrasound examination for postpartum stress urinary incontinence(SUI) in primipara. Method: A total of 86 primiparas who gave birth in Ganzhou People’s Hospital from January 2020 to April 2022 and returned to the hospital for re-examination 42 d after delivery were selected, all parturients underwent pelvic floor ultrasound examination, according to the occurrence of SUI, the patients were divided into SUI group(37 cases) and nonSUI group(49 cases). The maximal Valsalva maneuver and the anterior chamber structure parameters at rest were compared between the two groups of primiparas, and their diagnostic value for SUI were analyzed. Result: During the maximum Valsalva maneuver, the bladder neck and inferior margin of pubic symphysis(BNS) in the SUI group was lower than that in the non-SUI group, and the bladder neck descent(BND) and urethral rotation angle(URA) in the SUI group were higher than those in the non-SUI group(P<0.05). At rest, the bladder neck position(BNP) in the SUI group was lower than that in the non-SUI group, and the posterior bladder urethral angle(PUA) in the SUI group was higher than that in the non-SUI group(P<0.05). The receiver operating curve(ROC) curve was drawn, and the results showed that the area under the curve(AUC) of BNS, BND, URA, BNP, PUA and combined detection for diagnosing primipara SUI were 0.870, 0.871, 0.837, 0.804, 0.752, 0.927, respectively, has certain diagnostic value.Conclusion: The anterior chamber structure parameters measured by pelvic floor ultrasound can early detect the abnormal pelvic floor tissue structure in primipara, and have high diagnostic value for SUI in primipara.
