CT诊断联合超声血流参数对乳腺良恶性结节诊断的价值

Value of CT Diagnosis Combined with Ultrasonic Blood Flow Parameters in the Diagnosis of Benign and Malignant Breast Nodule

摘要 目的:探究乳腺结节鉴别诊断中CT诊断与超声血流参数联合应用的临床价值。方法:选取2019年6月-2021年6月泰安市肿瘤防治院收治的100例乳腺结节患者,所有患者结节的性质均经病理诊断证实。对本组患者的诊断资料进行回顾性分析,分析超声血流参数及CT诊断结果联合应用对乳腺结节性质的鉴别诊断效能。结果:(1)超声血流参数对比显示,恶性结节的搏动指数(PI)、收缩期峰值血流速度(PSV)、阻力指数(RI)均明显高于良性结节(P<0.001);(2)CT参数对比显示,恶性结节的血容量(BV)、血流量(BF)、表面通透性(PS)均明显高于良性结节(P<0.001);(3)CT形态学特征对比显示,恶性组的形态分叶率、非均匀强化率、边缘毛刺率均明显高于对照组(P<0.001);(4)CT、超声联合检查的敏感度、特异度均高于单独使用CT检查、超声检查。结论:良性、恶性结节的CT参数、形态学特征及超声血流参数存在明显差异,两种检查方法联合应用,有助于提高乳腺结节定性诊断的准确率。 Objective: To explore the clinical value of CT diagnosis and ultrasonic blood flow parameters in the differential diagnosis of breast nodules. Method: A total of 100 patients with breast nodules admitted to Taian Cancer Control Hospital from June 2019 to June 2021 were selected, and the nature of nodules in all patients was confirmed by pathological diagnosis. The diagnostic data of this group of patients were retrospectively analyzed to analyze the differential diagnosis efficacy of the combined application of ultrasonic blood flow parameters and CT diagnosis results on the nature of breast nodules. Result:(1) The pulse index(PI), peak systolic flow velocity(PSV)and resistance index(RI) of malignant nodules were significantly higher than those of benign nodules(P<0.001).(2) Comparison of CT parameters showed that the blood volume(BV), blood flow(BF) and surface permeability(PS) of malignant nodules were significantly higher than those of benign nodules(P<0.001).(3) Morphological characteristics of CT showed that the morphological lobulation rate, non-uniform enhancement rate and marginal burr rate of malignant group were significantly higher than those of control group(P<0.001).(4) The sensitivity and specificity of CT and ultrasound combined examination were higher than that of CT and ultrasound alone.Conclusion: There are significant differences in CT parameters, morphological characteristics and ultrasonic blood flow parameters between benign and malignant nodules. The combined application of the two examination methods can improve the accuracy of qualitative diagnosis of breast nodules.
