原发性高血压患者血浆PRA、Ang、Hcy、ET-1、NO的表达变化及其与血压分级的关系 被引量:2

摘要 目的:探究及分析原发性高血压患者血浆肾素活性(PRA)、血管紧张素(Ang)、同型半胱氨酸(Hcy)、内皮素-1(ET-1)、一氧化氮(NO)的表达变化及其与血压分级的关系。方法:选取2019年1月-2021年6月佳木斯市中心医院收治的82例原发性高血压患者,根据血压分级将其分为A组(高血压3级)27例、B组(高血压2级)27例及C组(高血压1级)28例,选取同时期的27例健康同龄者为D组。检测及比较四组的血浆PRA、Ang、Hcy、ET-1及NO水平及阳性率;采用Spearman秩相关分析血浆PRA、Ang、Hcy、ET-1及NO与原发性高血压患者血压分级的关系。结果:A组、B组及C组的PRA、Ang、Hcy及ET-1均高于D组,且A组及B组均高于C组,A组均高于B组,A组、B组及C组的NO均低于D组,A组及B组均低于C组,A组低于B组(P<0.05)。A组、B组及C组的血浆PRA、Ang、Hcy、ET-1及NO阳性率均高于D组,A组及B组均高于C组,A组均高于B组(P<0.05)。Spearman秩相关分析显示,血浆PRA、Ang、Hcy及ET-1与原发性高血压患者的血压分级均呈正相关,而血浆NO则与原发性高血压患者的血压分级呈负相关(P<0.05)。结论:原发性高血压患者的血浆PRA、Ang、Hcy、ET-1及NO表达显著异常,且与血压分级有密切的关系,对于原发性高血压的诊断及分级有较高的临床价值。 Objective: To investigate and analyze the expression change of plasma renin activity(PRA),angiotensin(Ang), homocysteine(Hcy), endothelin-1(ET-1) and nitric oxide(NO) of patients with essential hypertension and their correlation with blood pressure classifications. Method: A total of 82 patients with essential hypertension admitted to Jiamusi Central Hospital from January 2019 to June 2021 were selected, they were divided into group A(grade 3 hypertension) with 27 cases, group B(grade 2 hypertension) with 27 cases and group C(grade 1 hypertension) with 28 cases according to blood pressure classification, and 27 healthy patients of the same age during the same period were selected as group D. Plasma levels and positive rates of PRA, Ang, Hcy, ET-1 and NO were detected and compared among the four groups;Spearman rank correlation was used to analyze the relationship between plasma PRA, Ang, Hcy, ET-1 and NO and blood pressure classification in patients with essential hypertension. Result: The PRA, Ang, Hcy and ET-1 levels in the group A, group B and group C were higher than those in the group D, and those in the group A and group B were higher than those in the group C, and those in the group A were higher than those in group B, the NO levels in the group A, group B and group C were lower than those in the group D, the levels in the group A and group B were lower than those in the group C, and that in the group A was lower than that in the group B(P<0.05). The plasma positive rates of PRA, Ang, Hcy, ET-1 and NO in group A, group B and group C were higher than those in the group D, those in the group A and group B were higher than those in the group C, and those in the group A were higher than those in the group B(P<0.05). Spearman rank correlation analysis showed that plasma PRA, Ang, Hcy and ET-1 were positively correlated with the blood pressure classification of essential hypertension patients, while plasma NO was negatively correlated with the blood pressure classification of essential hypertension patients(P<0.05). Conclusion: The plasma PRA, Ang, Hcy, ET-1 and NO of patients with essential hypertension show abnormal expression, and they have close relationship with blood pressure classifications of essential hypertension, so the clinical value for diagnosis and classifications of patients with essential hypertension are high.
