摘要
目的探讨左甲状腺素片治疗妊娠期亚临床甲状腺功能减退症的效果及对妊娠结局的影响。方法抽取2021年1月至12月周口市妇幼保健院收治的64例妊娠期亚临床甲状腺功能减退患者作为观察对象,按照计算机分组法将其分为对照组和观察组,每组32例。对照组予以常规治疗,观察组予以左甲状腺素钠片治疗。比较两组治疗效果、甲状腺功能[促甲状腺激素(TSH)、游离三碘甲状腺原氨酸(FT3)和游离甲状腺素(FT4)]、血脂[总胆固醇(TC)、三酰甘油(TG)和低密度脂蛋白胆固醇(LDL-C)]水平、不良妊娠结局及新生儿结局。结果观察组总有效率(93.75%,30/32)高于对照组(68.75%,22/32),P<0.05。治疗后,观察组TSH、FT3、FT4变化幅度均优于对照组(P均<0.05);观察组TC、TG、LDL-C指标变化幅度均优于对照组(P均<0.05)。观察组早产、妊娠高血压综合征、前置胎盘、贫血、剖宫产、产后出血发生率均低于对照组(P均<0.05)。观察组新生儿总不良结局发生率(15.62%,5/32)低于对照组(40.62%,13/32),P<0.05。结论甲状腺素片治疗妊娠期亚临床甲状腺功能减退的效果较好,可改善患者的甲状腺功能指标、降低不良妊娠结局,确保母婴健康。
Objective To investigate the effect of levothyroxine sodium tablets on subclinical hypothyroidism in pregnancy and its influence on pregnancy outcome.Methods A total of 64 patients with subclinical hypothyroidism in pregnancy admitted to Zhoukou Maternal and Child Health Hospital from January 2021 to December 2021 were selected as the observation objects,and they were divided into control group and observation group according to the computer grouping method,with 32 cases in each group.The control group was given routine treatment,and the observation group was given levothyroxine sodium tablets.The therapeutic effect,thyroid function assessed by thyroid stimulating hormone(TSH),free triiodothyronine(FT3)and free thyroxine(FT4),blood lipid level including total cholesterol(TC),triglyceride(TG)and low-density lipoprotein cholesterol(LDL-C),adverse pregnancy outcome and neonatal outcome were compared between the two groups.Results The total effective rate of the observation group(93.75%,30/32)was higher than that of the control group(68.75%,22/32),P<0.05.After treatment,the changes of TSH,FT3 and FT4 in the observation group were better than those in the control group(all P<0.05);the changes of TC,TG and LDL-C in the observation group were better than those in the control group(all P<0.05).The incidence of premature delivery,pregnancy induced hypertension,placenta previa,anemia,cesarean section and postpartum hemorrhage in the observation group were lower than those in the control group(all P<0.05).The total incidence of adverse outcomes in the study group(15.62%,5/32)was lower than that in the control group(40.62%,13/32),P<0.05.Conclusions Thyroxine tablet has a good curative effect on subclinical hypothyroidism during pregnancy,which can improve the thyroid function index,reduce the adverse pregnancy outcome and ensure the health of the mother and infant.
