Willis覆膜支架治疗高流量直接颈内动脉海绵窦瘘的疗效分析

摘要 目的探讨Willis覆膜支架治疗高流量直接颈内动脉海绵窦瘘(CCF)的疗效。方法回顾性分析2015年8月至2019年8月采用Willis覆膜支架治疗的8例高流量直接CCF的临床资料。结果8例共置入Willis覆膜支架8枚,均一次成功置入支架。7例球囊扩张后支架贴壁良好,1例支架释放过程发生移位、远心端贴壁不良。术后即刻血管造影显示病变完全消失6例,3次球囊扩张后仍有少量内漏1例,3次球囊扩张后仍有明显内漏1例。8例术后随访6~25个月,平均(13±5.6)个月;8例CCF均无复发,责任动脉通畅;1例术后遗留的少量内漏自行闭塞;1例遗留的明显内漏未消失,但症状稳定。结论Willis覆膜支架治疗高流量直接CCF是一种可行的、有效的方法。 Objective To explore the outcomes of Willis covered stents foRthe treatment of patients with high-flow direct carotid cavernous fistulas(CCFs).Methods The clinical data of 8 patients with high-flow direct CCFs who were treated by Willis covered stents from August 2015 to August 2019 were analyzed retrospectively.Results All the patients were successfully treated with Willis covered stents.The stents were well-adherent afteRballoon dilation in 7 patients,while the stent was displaced during release and poorly adherent at the distal end in 1 patient.Immediate postoperative angiography showed complete disappearance of the fistulas in 6 patients,light internal leakage afteR3 times balloon dilation in 1 patient,and heavy internal leakage afteR3 times balloon dilation in 1 patient.The 8 patients were followed up foR6~25 months,with an average of(13±5.6)months.There was no recurrence of CCF in 8 patients,and the responsible arteries were patency.One patient with light internal leakage afteRsurgery was self-occluded during follow-up.One patient with heavy internal leakage did not disappeaRduring follow-up,but the symptoms were stable.Conclusions Willis coated stent is a feasible and effective method foRtreating patients with high flow direct CCFs.
