摘要
目的探讨醒脑开窍法针刺联合活血通络汤对脑梗死患者脑氧代谢及炎症因子水平的影响。方法选取2017年1月至2021年1月我院收治的360例脑梗死患者,将其随机分为对照组和观察组,各180例。对照组给予规范化西医治疗,观察组在对照组基础上给予醒脑开窍法针刺联合活血通络汤治疗。比较两组的脑氧代谢指标、炎症因子水平、脑血流动力学指标及生活质量。结果治疗后,两组的动静脉血氧含量差(Da-jvO_(2))、氧摄取率(ERO_(2))均降低,颈静脉血氧饱和度(SjvO_(2))均升高,且观察组优于对照组(P<0.05)。治疗后,两组的白细胞介素-6(IL-6)、基质金属蛋白酶-9(MMP-9)、可溶性血管细胞黏附分子-1(sVCAM-1)水平均降低,且观察组低于对照组(P<0.05)。治疗后,两组的基底动脉舒张末期血流速度(Vd)、收缩峰值血流速度(Vs)、平均血流速度(Vm)均升高,且观察组高于对照组(P<0.05)。治疗后,观察组的美国国立卫生院卒中量表(NIHSS)、改良Rankin量表(mRS)评分低于对照组,Barthel指数(BI)评分高于对照组(P<0.05)。结论醒脑开窍法针刺联合活血通络汤治疗脑梗死可提高治疗效果,改善脑氧代谢及炎症反应有较好改善效果,提高生活质量,值得临床应用。
Objective To explore the effects of Xingnao Kaiqiao acupuncture combined with Huoxue Tongluo decoction on cerebral oxygen metabolism and inflammatory factors levels in patients with cerebral infarction.Methods A total of 360 patients with cerebral infarction admitted in our hospital from January 2017 to January 2021 were selected and randomly divided into control group and observation group,with 180 cases in each group.The control group was treated with standardized western medicine,and the observation group was treated with Xingnao Kaiqiao acupuncture combined with Huoxue Tongluo decoction on the basis of the control group.The cerebral oxygen metabolism index,inflammatory factor level,cerebral hemodynamics index and quality of life were compared between the two groups.Results After treatment,the arterio-venous oxygen content difference(Da-jvO_(2))and oxygen extraction ratio(ERO_(2))in the two groups decreased,the jugularn venous oxygen saturation(SjvO_(2))increased,and those in the observation group were better than the control group(P<0.05).After treatment,the levels of interleukin-6(IL-6),matrix metalloproteinase-9(MMP-9)and soluble vascular cell adhesion molecule-1(sVCAM-1)in the two groups decreased,and those in the observation group were lower than the control group(P<0.05).After treatment,the basilar artery end-diastolic blood flow velocity(Vd),peak systolic blood flow velocity(Vs)and mean blood flow velocity(Vm)in the two groups increased,and those in the observation group were higher than the control group(P<0.05).After treatment,the scores of the National Institutes of Health Stroke Scale(NIHSS)and Modified Rankin Scale(mRS)in the observation group were lower than those in the control group,and the Barthel Index(BI)score was higher than that in the control group(P<0.05).Conclusion Xingnao Kaiqiao acupuncture combined with Huoxue Tongluo decoction in the treatment of cerebral infarction can ameliorate the therapeutic effect,ameliorate the cerebral oxygen metabolism and inflammatory reaction,and improve the quality of life,which is worthy of clinical application.
