摘要
目的比较小儿肱骨髁上骨折实施闭合穿针内固定术、中医手法复位加石膏外固定的临床疗效。方法方便选取2010年7月—2021年7月聊城市中医医院收治的58例小儿肱骨髁上骨折患儿,以治疗方式不同分为两组。保守组给予中医手法复位+石膏外固定治疗(19例);手术组给予闭合穿针内固定术治疗(39例)。比较两组临床疗效。结果保守组治疗总有效率(89.47%)高于手术组,差异有统计学意义(χ^(2)=4.811,P=0.028);治疗后,保守组肘关节活动范围(87.54±9.68)分、活动能力(88.59±10.14)分、稳定性(91.35±8.26)分均高于手术组,差异有统计学意义(t=3.270、3.444、3.786,P<0.05);保守组并发症发生率(10.53%)低于手术组,差异有统计学意义(χ^(2)=4.116,P=0.042);保守组治疗后生活质量评分均高于手术组,差异有统计学意义(P<0.05)。结论中医手法复位+石膏外固定治疗小儿肱骨髁上骨折效果较好。
Objective To compare the clinical effects of closed needle internal fixation,traditional Chinese medicine manual reduction and plaster external fixation for supracondylar fractures of the humerus in children.Methods A total of 58 children with supracondylar fractures of the humerus admitted to Liaocheng Hospital of Traditional Chinese Medicine from July 2010 to July 2020 were conveniently selected and divided into two groups according to different treatment methods.The conservative group were treated with TCM manual reduction and plaster external fixation(19 cases),the operation group were treated with closed needle internal fixation(39 cases).The clinical efficacy of the two groups was compared.Results The total treatment effective rate of conservative group was 89.47%higher than that of operation group,the difference was statistically significant(χ^(2)=4.811,P=0.028);after treatment,the elbow joint range of motion(87.54±9.68)points,mobility(88.59±10.14)points,and stability(91.35±8.26)points in the conservative group were higher than those in the operation group,the difference was statistically significant(t=3.270,3.444,3.786,P<0.05).The complication rate of conservative group(10.53%)was lower than that of operation group,the difference was statistically significant(χ^(2)=4.116,P=0.042).And the quality of life scores in the conservative group after treat⁃ment were higher than those in the surgery group,the difference was statistically significant(P<0.05).Conclusion TCM manual reduction combined with plaster external fixation has a good effect in the treatment of supratarsal frac⁃tures of the humerus in children.
