摘要
目的探讨氟代脱氧葡萄糖(18F-FDG)PET/CT上肺部磨玻璃结节(GGN)最大标准摄取值(SUVmax)与纵隔血池SUVmax值的比值(即摄取比)对不同浸润程度GGN型肺腺癌的诊断价值。方法回顾性分析2019年1月至2021年12月于本院行18F-FDG PET/CT检查的101例GGN型肺腺癌患者(129个GGN病灶)的影像资料,记录患者病灶SUVmax值及纵隔血池SUVmax值,并计算出二者摄取比。以术后病理结果为金标准,根据肺腺癌浸润程度及临床意义不同将所有GGN分为两组:浸润前-微浸润腺癌组、浸润性腺癌组。通过绘制受试者工作特征(ROC)曲线分别评估SUVmax值、摄取比对不同浸润程度GGN型肺腺癌的预测价值。结果相比浸润前-微浸润腺癌组,浸润性腺癌组的SUVmax值及摄取比均明显升高(P<0.001)。ROC曲线分析发现:摄取比对诊断不同浸润程度GGN型肺腺癌的灵敏度、特异度及准确率分别为80.36%、95.89%、89.15%,均高于SUVmax值的诊断效能(灵敏度、特异度及准确率分别为76.79%、89.04%、83.72%)。结论18F-FDG PET/CT摄取比对GGN型肺腺癌的浸润程度有较高的评估价值,且诊断效能优于SUVmax值。
Objective To investigate the diagnosis value of the ratio of SUVmaxof pulmonary ground-glass nodule to SUVmaxof mediastinal blood pool on18F-FDG PET/CT(SUV ratio)in GGN lung adenocarcinoma with different degrees of invasion.Methods The imaging data of 101 patients with GGN lung adenocarcinoma(129 cases of GGN lesions)who underwent18F-FDG PET/CT examination in our hospital from January 2019 to December 2021 were analyzed retrospectively.The SUVmaxof the lesions and the SUVmaxof the mediastinal blood pool were recorded,and the ratio of the two(SUV ratio)was calculated.According to the infiltration degree of lung adenocarcinoma and clinical significance,GGN was divided into two groups:pre invasion-minimally invasive adenocarcinoma group、invasive adenocarcinoma group.The predictive value of SUVmaxand SUV ratio for GGN lung adenocarcinoma with different degrees of invasion were evaluated by drawing receiver operating characteristic(ROC)curves.Results SUVmaxand SUV ratio in invasive adenocarcinoma group were significantly higher than those in pre invasive-minimally invasive adenocarcinoma group(P<0.001).ROC curve analysis showed that the sensitivity,specificity and accuracy of SUV ratio in the diagnosis of different degrees of invasion of GGN lung adenocarcinoma were 80.36%,95.89%and 89.15%respectively,which was higher than that of SUVmax(sensitivity,specificity and accuracy were 76.79%,89.04%and 83.72%respectively).Conclusion18F-FDG PET/CT SUV ratio has higher value in evaluating the degree of invasion of GGN lung adenocarcinoma,and the diagnostic efficiency is better than SUVmax.
