摘要
目的探究能谱纯化技术联合高级模拟迭代重组技术(ADMIRE)(ADMIRE 1~5级)对磨玻璃结节复查患者行胸部超低剂量CT图像质量的影响。方法纳入31例磨玻璃结节复查的患者。应用第三代双源CT扫描仪,采用Sn100 kV、自动管电流(预设值200 mAs)扫描方案。所得原始数据采用滤波反投影法(FBP)、ADMIRE 1~5级重组肺窗(BI57)、纵隔窗(Br40)图像,重组层厚1.0mm,层间距0.7 mm。测量并记录各组图像客观参数、计算信噪比(SNR)及对比噪声比(CNR)。采用5分法分别对肺窗、纵隔窗图像及磨玻璃结节进行主观评估。对比分析6组图像的定性及定量指标。结果FBP组、ADMIRE 1组、ADMIRE 2组、ADMIRE 3组、ADMIRE 4组、ADMIRE 5组肺窗图像噪声值分别为(124.35±23.16)HU、(107.6±19.87)HU、(90.95±16.48)HU、(74.53±13.23)HU、(58.66±11.47)HU、(43.26±7.21)HU;ADMIRE1~5组较FBP组噪声分别降低13.44%、26.78%、39.93%、52.70%、64.89%(F=4993.603,P=0.000)。纵隔图像噪声值分别为(47.56±5.93)HU、(42.78±5.36)HU、(37.79±4.95)HU、(32.99±4.52)HU、(27.84±3.96)HU、(21.77±3.46)HU,ADMIRE 1~5组纵隔图像噪声较FBP组分别降低10.05%、20.58%、30.70%、41.53%、54.34%(F=2869.284,P=0.000),SNR较FBP组分别升高12.11%、25.63%、43.86%、70.14%、118.43%(F=791.358,P=0.000),CNR分别升高22.33%、30.81%、52.44%、80.03%、139.58%(F=9.547,P=0.000),差异具有统计学意义。ADMIRE 3~5组肺窗、ADMIRE 4~5组纵隔窗、ADMIRE 2~5组磨玻璃结节评分均能满足临床诊断需求,ADMIRE 5组主观评分最高。31例患者的平均有效辐射剂量(ED)为(0.22±0.07)mSv。结论ADMIRE重组算法可明显降低能谱纯化技术图像噪声、提高图像质量,其中ADMIRE 5级降噪能力及提高图像质量效果最为显著。
Objective To investigate the effect of selected photon shield technique combined with the ADMIRE reconstruction algorithm(ADMIRE levels 1 to 5)on the quality of ultra-low dose CT images of the chest performed in patients with a review of ground glass nodules.Methods 31 patients were enrolled.A third-generation dual-source CT scanner with tube voltage Sn100 kV and automatic tube current(preset value 200 mAs)scanning protocol.The raw data obtained were reconstructed using FBP,ADMIRE level 1-5,lung window(BI57)and mediastinal window(Br40)with a layer thickness of 1.0 mm and a layer spacing of 0.7 mm.The image noise,signal-to-noise ratio(SNR)and contrast-to-noise ratio(CNR)were measured and recorded for each group.The image of lung window,mediastinal window and ground glass nodules were subjectively assessed using the 5-point method.The qualitative and quantitative parameters of the 6 groups were compared and analyzed.Results The lung window image noise values of FBP group,ADMRE 1 group,ADMRE 2 group,ADMRE 3 group,ADMRE 4 and ADMRE 5 group were(124.35±23.16)HU、(107.6±19.87)HU、(90.95±16.48)HU、(74.53±13.23)HU、(58.66±11.47)HU、(43.26±7.21)HU.Compared with FBP group,the lung window image noise of ADMIRE 1-5 group was reduced by 13.44%、26.78%、39.93%、52.70%、64.89%(F=4993.603,P=0.000).The mediastinal window image noise values were(47.56±5.93)HU、(42.78±5.36)HU、(37.79±4.95)HU、(32.99±4.52)HU、(27.84±3.96)HU、(21.77±3.46)HU.Compared with FBP group,mediastinal image noise of ADMIRE 1-5 group was decreased by 10.05%,20.58%,30.70%,41.53%、54.34%(F=2869.284,P=0.000),and SNR was increased by 12.11%、25.63%、43.86%、70.14%、118.43%(F=791.358,P=0.000),CNR increased by 22.33%、30.81%、52.44%、80.03%、139.58%(F=9.547,P=0.000).The lung window in ADMIRE 3-5 groups,the mediastinal window in ADMIRE 4-5 groups,and the ground glass nodule score in ADMIRE 2-5 groups all met the clinical diagnostic needs,and the subjective score in ADMIRE 5 group was the highest.Mean ED of 31 patients was(0.22±0.07)mSv.Conclusion The ADMIRE reconstruction algorithm can significantly reduce the noise and improve the image quality of the selected photon shield technique,with the ADMIRE level 5 algorithm having the most significant noise reduction and image quality improvement effect.
