晚发性新生儿肺部感染常见细菌及应用头孢哌酮钠舒巴坦钠治疗的效果分析 被引量:1

摘要 目的:分析引发新生儿肺部感染的常见细菌,探讨头孢哌酮钠舒巴坦钠的治疗效果。方法:选取2017年3月-2021年2月北京爱育华妇儿医院收治的120例新生儿感染性肺炎患儿作为研究对象,根据治疗方式的不同分为参照组与研究组,各60例。两组患儿在未使用抗生素治疗前进行痰培养及药敏试验,研究组给予头孢哌酮钠舒巴坦钠进行治疗,参照组给予头孢呋辛钠进行治疗。分析两组患儿感染细菌情况,比较两组临床指标、血气指标、炎性因子水平及不良反应发生情况。结果:120例患儿感染细菌包括大肠埃希菌、肺炎克雷伯菌、金黄色葡萄球菌、表皮葡萄球菌。研究组气促消失时间、吐沫消失时间、肺部啰音消失时间、平均住院时间均短于参照组,差异有统计学意义(P=0.000)。治疗后,研究组动脉血氧分压高于参照组,动脉血二氧化碳分压水平低于参照组,差异有统计学意义(P=0.000)。治疗后,研究组C反应蛋白、降钙素原、γ干扰素、白细胞总数水平均低于对照组,差异有统计学意义(P=0.000)。研究组不良反应发生率低于参照组,差异有统计学意义(P=0.005)。结论:应用头孢哌酮钠舒巴坦钠对感染性肺炎新生儿进行治疗,可以尽快改善患儿各种临床症状,缩短住院时间,促进血气指标及炎性因子指标改善。 Objective:To analyze the common bacteria that induce neonatal pulmonary infection and to explore the treatment effect by using cefoperazone sodium and sulbactam sodium.Methods:A total of 120 infants with neonatal infectious pneumonia who were admitted to Beijing Aiyuhua Women’s and Children’s Hospital from March 2017 to February 2021 were selected as the study subjects,and they were divided into a reference group and a study group according to the differences in treatment methods,with 60cases in each group.Sputum culture and drug sensitivity test were performed before antibiotic treatment in both groups,The study group was treated with cefoperazone sodium and sulbactam sodium,and the reference group was treated with cefuroxime sodium.The bacterial infection of infants in two groups was analyzed.The clinical indicators,blood gas indicators,levels of inflammatory factors and occurrence of adverse reactions in two groups were compared.Results:The 120 children were infected with bacteria,including Escherichia coli,Klebsiella pneumoniae,Staphylococcus aureus,and Staphylococcus epidermidis.The time to disappearance of shortness of breath,disappearance of saliva spitting,disappearance of pulmonary rales,and mean length of hospital stay in the study group were all shorter than those in the reference group,and the differences were statistically significant(P=0.000).After treatment,the PaO2level in the study group was higher than that in the reference group,and the PaCO2level was lower than that in the reference group,and the difference was statistically significant(P=0.000).After treatment,the levels of CRP,PCT,IFN-γ and WBC in the study group were lower than those in the reference group,and the difference was statistically significant(P=0.000).The incidence of adverse reactions in the study group was lower than that in the reference group,and the difference was statistically significant(P=0.005).Conclusion:The application of cefoperazone sodium and sulbactam sodium in the treatment of neonatal pneumonia can improve various clinical symptoms of infants as soon as possible,shorten the length of hospital stay,and promote the improvement of blood gas indexes and inflammatory factor indexes.
