摘要
目的探讨循证护理在点式直线偏振光疼痛治疗仪治疗颞下颌关节紊乱病中的应用效果。方法将112例颞下颌关节紊乱病患者按简单随机分组法分为观察组、对照组,各56例。对照组给予常规护理干预,观察组在对照组基础上给予循证护理,观察1个月。比较两组临床疗效及治疗前后疼痛程度、咬肌表面肌电图指标、生活质量。结果观察组总有效率为87.50%,对照组为71.43%,观察组显著高于对照组(P<0.05)。治疗后两组关节区、肌肉区疼痛评分均较治疗前显著降低(P<0.01),观察组显著低于对照组(P<0.01)。治疗后两组咬肌表面肌电图的紧咬棉棒5 s、牙尖交错位最大自主紧咬和下颌姿势位测试结果均显著优于治疗前(P<0.01),观察组显著优于对照组(P<0.01)。治疗后两组世界卫生组织生存质量简明量表总分及社会关系领域、心理领域、环境领域、生理领域评分均较治疗前显著升高(P<0.01),观察组显著高于对照组(P<0.01)。结论在点式直线偏振光疼痛治疗仪治疗中,对颞下颌关节紊乱病患者予以循证护理,能提高临床疗效,改善患者表面肌电图,减轻患者疼痛,提高其生活质量,值得在临床推广应用。
Objective To analyze the application value of evidence-based nursing in the treatment of temporomandibular disorder(TMD)with super lizer.Methods A total of 112 TMD patients were divided into observation group and control group according to simple randomization,with 56 patients in each group.The control group was given routine nursing intervention,and the observation group was given evidence-based nursing on the basis of the control group for 1 month.The clinical efficacy,pain degree,masseter surface electromyography index and quality of life were compared between the two groups.Results The total effective rate of the observation group was 87.50%,which was significantly higher than that in the control group(71.43%)(P<0.05).After treatment,the pain scores in the joint area and muscle area of the two groups were significantly lower than those before treatment(P<0.01),and the observation group was significantly lower than the control group(P<0.01).After treatment,the test results of clenching the cotton swab for 5 s,maximum autonomous biting in cusp interlacing position and mandibular postural position in both groups were significantly better than those before treatment(P<0.01),and the observation group was significantly better than the control group(P<0.01).After treatment,the total score of World Health Organization quality of life brief scale(WHOQOL-BREF)and the scores of social relation field,psychological domain,environmental area,physiological field of the two groups were significantly higher than those before treatment(P<0.01),and the observation group was significantly higher than the control group(P<0.01).Conclusions The application of evidence-based nursing has a good effect in the treatment of TMD by super lizer,which can improve patients'surface electromyography,relieve patients'pain and improve their quality of life,and is worthy of clinical application.
