摘要
目的探讨FOCUS-PDCA模式在静脉用药调配中心(PIVAS)高警示药品管理中的应用效果。方法运用FOCUS-PDCA模式管理某院PIVAS高警示药品,梳理高警示药品品种知晓、日常存储、标识管理、医嘱审核、流程管理、药品转运、人员培训等,并进行优化。收集FOCUS-PDCA模式干预前(2020年11月至2021年7月)和干预后(2021年8月至2022年4月)的医嘱992条和1027条,比较高警示药品分区存储规范率,药品品种知晓、标识管理、存储转运知晓、流程落实4个认知执行度,审方事故、摆退药事故、转运事故3个相关事故发生情况。结果干预后,高警示药品平均存储规范率为100.00%,显著高于干预前的97.18%(P<0.05);药品品种知晓、标识管理、存储转运知晓、流程落实的评分均较干预前显著升高(P<0.05);相关事故发生率为4.03%,显著低于干预前的21.81%(P<0.01)。结论FOCUS-PDCA模式能提高高警示药品的管理质量,减少相关事故,保障患者的用药安全。
Objective To investigate the application effect of FOCUS-PDCA mode in the high-alert medication management in the pharmacy intravenous admixture services(PIVAS)of a hospital.MethodsThe FOCUS-PDCA mode was used to manage the high-alert medications in PIVAS of a hospital,the information of high-alert medications on the variety awareness,daily storage,labeling management,medical-order review,process management,drug transport and personnel training were sorted,and the above management contents were optimized.A total of 992 medical orders before the intervention of FOCUS-PDCA mode(November2020 to July 2021)and 1027 medical orders after the intervention of FOCUS-PDCA mode(August 2021 to April 2022)were collected to compare the standardized rate of partitioned storage of high-alert medications,four cognitive execution levels on the awareness of medication variety,labeling management,awareness of storage and transport and implementation of process,the incidence of three related accidents including prescription-review accidents,medication-dispensing and medication-repercussion accidents and transport accidents.ResultsAfter the intervention,the average standardized rate of storage of high-alert medications was 100.00%,which was significantly higher than 97.18%before intervention(P<0.05).After the intervention,the scores of awareness of medication variety,labeling management,awareness of storage and transport and implementation of process were significantly higher than those before the intervention(P<0.05).After the intervention,the incidence of related accidents was4.03%,which was significantly lower than 21.81%before the intervention(P<0.01).ConclusionFOCUS-PDCA mode can improve the management quality of high-alert medications,reduce the related accidents,and ensure the medication safety of patients.
