基于数据挖掘技术的针刺治疗运动性失语选穴规律探析 被引量:1

摘要 目的:通过对针刺治疗运动性失语的相关RCT文献进行数据挖掘探析选穴规律,为临床和科研提供参考和借鉴。方法:通过检索知网(CNKI)、万方(Wanfang)、维普(Vip)、中国生物医学数据库(CBM)、Pubmed、Web of science及Embase等中英文数据库中有关针刺治疗运动性失语的文献,通过纳入、排除标准对文献进行筛选,最终将符合文献中的数据提取到Excel表格中进行汇总统计并描述性分析,运用IBM SPSS Modeler18.0及IBM SPSS Statistics 25.0软件对纳入的穴位处方分别进行关联规则分析和聚类分析。结果:最终纳入文献101篇,穴位处方101条,腧穴数99个,总频次528次。统计分析结果显示,频次最高的腧穴、经脉、分布部位及特定穴分别是廉泉、督脉、头颈部和五腧穴;关联规则表显示支持度最高的穴对组合为“玉液-金津”,复杂网络图显示针刺治疗运动性失语的核心处方穴对为“廉泉-金津-玉液-哑门-百会-通里”。聚类分析结果得出:以23为标度可得出4个聚类群,分别是“金津-玉液-风池-合谷-顶颞前斜线”“言语一区-哑门-通里-言语二区-言语三区-运动区”“心俞-神庭-四神聪-百会”“三阴交-内关-水沟-旁廉泉-廉泉”。结论:针刺治疗运动性失语选穴以局部舌针取穴为主,结合头针言语区及体针脏腑相关(舌为心之苗),注重五腧穴及俞原配穴法的应用,体现“通瘀活络,强肾助心”的治则。 Objective:To explore the rule of points selection based on data mining of the relevant RCT literature about acupuncture treating motor aphasia,thus to provide reference for clinical and scientific researches.Methods:Literature on acupuncture treatment for motor aphasia were retrieved from the databases of CNKI,Wanfang,Vip,CBM,Pubmed,Web of science and Embase.The literature was screened by inclusion and exclusion criteria,and finally the data was input into an Excel table for summary statistics for descriptive analysis,and the included acupoint prescriptions were separately performed by using IBM SPSS Modeler18.0 and IBM SPSS Statistics 25.0 for association rule analysis and cluster analysis.Results:101 pieces of literature were finally included,involving 101 acupoint prescriptions and 99 acupoints,with a total frequency of 528 times.The statistical analysis results showed that acupoint,meridian,points locations and specific points with highest frequency were Lianquan(RN23),DU meridian,head and neck,and five-Shu points,respectively.The association rule table showed that the most frequent acupoint pair was Jinjin(EX-HN1)-Yuye(EX-HN2),and the complex network diagram showed that the core prescription of acupuncture treatment for motor aphasia was RN23-(EX-HN1)-(EX-HN2)-Yamen(DU15)-Baihui(DU20)-Tongli(HT5).The result of clustering analysis showed that 4 clusters could be obtained on a scale of 23,namely,(EX-HN1)-(EX-HN2)-Fengchi(GB20)-Hegu(LI4)-paratotemporal anterior diagonal line;speech area one-DU15-HT5-speech area two-speech area three-motor area,Xinyu(BL15)-Shenting(DU24)-Sishencong(EX-HN1)-DU20,and Sanyinjiao(SP6)-Neiguan(PC6)-Shuigou(DU26)-Panglianquan-RN23.Conclusion:Acupuncture treatment for motor aphasia is mainly based on local tongue acupuncture,combined with scalp acupuncture speech areas and body acupuncture related to the viscera(the tongue is the seedling of Heart),paying attention to the application of five-Shu points and the Shu-Yuan matching,which manifests the principle of‘removing stasis and activating collaterals,strengthening Kidney and Heart’.
