摘要 目的探讨C反应蛋白(C-reactive protein,CRP)、降钙素原(procalcitonin,PCT)对儿童呼吸道细菌与肺炎支原体感染的诊断鉴别作用。方法选择2020年4月-2021年7月本院收治的儿童呼吸道感染患儿93例,根据感染类型不同分组,其中呼吸道细菌感染41例,肺炎支原体感染52例,均进行CRP、PCT检测,对两组CRP、PCT检测结果进行比较。结果呼吸道细菌感染组的CRP、PCT水平与肺炎支原体感染组比较有明显差异(P<0.05);呼吸道细菌感染组的PCT、CRP单一检测与联合检测阳性率分别为87.80%、80.49%、95.12%,较肺炎支原体感染组17.31%、53.85%、65.38%高(P<0.05);呼吸道细菌感染联合检测特异度、敏感度为93.28%、85.39%,肺炎支原体感染联合检测特异度、敏感度分别为84.25%、83.16%,联合检测的诊断特异度与敏感度明显较单一检测高(P<0.05)。结论CRP、PCT对儿童呼吸道细菌与肺炎支原体感染具有较高的诊断鉴别作用,值得推广。 Objective To investigate the role of C-reactive protein(CRP)and procalcitonin(PCT)in the diagnosis and differentiation of respiratory bacteria and Mycoplasma pneumoniae infection in children.Methods A total of 93 children with respiratory tract infection who were admitted from April 2020 to July 2021 were selected and grouped according to the type of infection,including 41 cases of respiratory tract bacterial infection,52 cases of Mycoplasma pneumoniae infection,CRP and PCT were detected,and the results of CRP and PCT were compared between the two groups.Results The levels of CRP and PCT in the bacterial infection group were significantly different from those in the Mycoplasma pneumoniae infection group(P<0.05);the positive rates of single detection and combined detection of PCT and CRP in bacterial infection group were 87.80%,80.49%,and 95.12%,respectively,which were higher than 17.31%,53.85%,and 65.38%in Mycoplasma pneumoniae infection group(P<0.05);the specificity and sensitivity of the combined detection of bacterial infection were 93.28%and 85.39%,and the specificity and sensitivity of the combined detection of Mycoplasma pneumoniae infection were 84.25%and 83.16%,respectively.The diagnostic specificity and sensitivity of combined detection were significantly higher than those of single detection(P<0.05).Conclusion CRP and PCT have a high diagnostic and differential effect on children’s respiratory tract bacteria and Mycoplasma pneumoniae infection,and are worthy of promotion.
