摘要
慢性阻塞性肺疾病(COPD)是一种慢性气道炎症疾病,发病机制主要认为是由慢性炎症反应、蛋白酶和抗蛋白酶失衡、氧化抗氧化失衡、气道重塑等过程引起的气道高黏液分泌、慢性细支气管炎和肺气肿等病理变化。缺氧诱导因子-1α(HIF-1α)是一种在低氧环境下激活,使机体适应低氧环境的转录调控因子,广泛存在于慢性缺氧细胞中。HIF-1α的激活对COPD的发病过程产生了重要影响,与COPD的发病机制密切相关,并且与COPD患者肺功能状况密切相关。近年来,中医药领域以HIF-1α信号通路为切入点,开展了大量的动物实验与临床试验,现代研究表明,中医药能够针对HIF-1α及与HIF-1α相关的多种信号通路产生多层次的影响。该文分析了中药单味、中药复方及中医适宜技术对COPD中HIF-1α表达影响的相关实验研究,帮助临床医生更全面地认识中医药干预HIF-1α的作用及其对COPD的治疗效果,推动相关研究,以便更好地探究其机制,更好地指导临床用药。
Chronic obstructive pulmonary disease(COPD)is a chronic airway inflammatory disease.Its pathogenesis is mainly believed to include pathological changes such as airway hypermucus secretion,chronic bronchiolitis and emphysema caused by chronic inflammatory reaction,imbalance between protease and antiprotease,imbalance between oxidation and antioxidation,airway remodeling,etc. Hypoxiainducible factors-1α(HIF-1α)is a kind of transcription factor that activates in hypoxia environment and makes the body adapt to the hypoxia environment,and widely exists in chronic hypoxia cells. The activation of HIF-1α plays an important role in the pathogenesis of COPD,which is closely related to the pathogenesis of COPD and the pulmonary function of COPD patients. In recent years,a large number of animal and clinical experiments have been carried out in the field of traditional Chinese medicine with the HIF-1αsignal pathway as the starting point,traditional Chinese medicine can produce multi-level effects on HIF-1α and the multiple signal pathways related to HIF-1α. In this paper,we analyzed the experimental study on the effect of single herbs,compound herbs and appropriate techniques of traditional Chinese medicine on the expression of HIF-1α in COPD,to help clinicians understand the role of traditional Chinese medicine in HIF-1α intervention and its therapeutic effect on COPD,and to promote related research,so as to better explore its mechanism and better guide clinical medication.
