甲状腺癌患者PLR、Apo A1、FT_(4)/FT_(3)水平变化及与疾病严重程度的相关性

Changes of PLA, Apo A1, FT_(4)/FT_(3) Levels in Patients with Thyroid Cancer and Their Correlation with Disease Severity

摘要 目的 探讨甲状腺癌患者血小板和淋巴细胞比值(PLR)、载脂蛋白A1(Apo A1)、血清游离甲状腺素(FT_(4))与游离三碘甲状腺原氨酸(FT_(3))比值(FT_(4)/FT_(3))变化及与疾病严重程度相关性。方法 选取洛阳市第六人民医院2018年3月至2021年3月收治的76例甲状腺癌患者纳入恶性组,同期选择医院收治的40例甲状腺良性病变患者纳入良性组。检测两组患者PLR、Apo A1、FT_(4)、FT_(3)及FT_(4)/FT_(3)变化,分析PLR、Apo A1、FT_(4)/FT_(3)与甲状腺癌疾病严重程度的相关性及分析PLR、Apo A1、FT_(4)/FT_(3)对甲状腺癌的诊断效能。结果 两组患者FT_(4)水平比较,差异无统计学意义(P>0.05);恶性组PLR、FT_(4)/FT_(3)高于良性组,Apo A1、FT_(3)低于良性组(P<0.05)。经Spearman相关分析显示,甲状腺癌疾病严重程度与PLR和FT_(4)/FT_(3)呈正相关(r=0.633、0.614,P<0.001),与Apo A1呈负相关(r=-0.578,P<0.001)。经受试者工作特征(ROC)曲线分析得出,PLR对甲状腺癌诊断的曲线下面积为0.774、特异度为87.5%、敏感度为59.2%,Apo A1对甲状腺癌诊断的曲线下面积为0.778、特异度为85.0%、敏感度为51.3%,FT_(4)/FT_(3)对甲状腺癌诊断的曲线下面积为0.813、特异度为65.0%、敏感度为90.8%,均有一定诊断价值(P<0.05)。结论 PLR、FT_(4)/FT_(3)在甲状腺癌中表达水平升高,Apo A1表达水平下降,可为评估甲状腺癌疾病严重程度提供参考。 Objective To investigate the changes of platelet-to-lymphocyte ratio(PLR), apolipoprotein A1(Apo A1), serum free thyroxine(FT_(4)) and free triiodothyronine(FT_(3)) ratio(FT_(4)/FT_(3)) in patients with thyroid cancer and their correlation with the severity of the disease.Methods A total of 76 patients with thyroid cancer admitted to the Sixth People’s Hospital of Luoyang from March 2018 to March 2021 were selected to be included in the malignant group, and 40 patients with benign thyroid diseases admitted to the hospital were selected to be included in the benign group at the same time. The changes of PLR, Apo A1, FT_(4), FT_(3)and FT_(4)/FT_(3)in the two groups of patients were detected. The correlations between PLR, Apo A1, FT_(4)/FT_(3)and the severity of thyroid cancer, and the diagnostic efficacy of PLR, Apo A1, FT_(4)/FT_(3)for thyroid cancer were analyzed.Results There was no difference in FT_(4)level between the two groups(P>0.05). PLR and FT_(4)/FT_(3)in malignant group were higher than those in benign group, and Apo A1 and FT_(3)were lower than those in benign group(P<0.05). Spearman correlation analysis showed that the severity of thyroid cancer was positively correlated with PLR and FT_(4)/FT_(3)(r=0.633, 0.614, P<0.001), and negatively correlated with Apo A1(r=-0.578, P<0.001). The analysis of receiver operating characteristic(ROC) curve showed that the area under the curve of PLR for thyroid cancer diagnosis was 0.774, specificity was 87.5%, sensitivity was 59.2%, the area under the curve of Apo A1 for thyroid cancer diagnosis was 0.778, specificity was 85.0%, sensitivity was 51.3%, and the area under the curve of FT_(4)/FT_(3)for thyroid cancer diagnosis was 0.813, specificity was 65.0%, sensitivity was 90.8%, all of which had certain diagnostic value(P<0.05).Conclusion The expression level of PLR and FT_(4)/FT_(3)in thyroid cancer increased, and the expression level of Apo A1 decreased, which can provide reference for evaluating the severity of thyroid cancer.
