摘要
目的探讨PDCA循环管理在骨科Ⅰ类切口手术患者抗菌药物预防中的应用效果。方法回顾性分析2022年1月至3月徐州仁慈医院的1399例Ⅰ类切口手术出院患者作为对照组,2022年4月至6月的984例Ⅰ类切口手术出院患者作为观察组。对照组患者采用常规管理,观察组患者采用PDCA循环管理。比较两组患者围手术期抗菌药物预防应用情况、骨科重点专科中抗菌药物预防使用率、围手术期抗菌药物预防用药疗程与感染率。结果观察组患者的抗菌药物预防使用率低于对照组,医师抗菌药物知识合格率、品种选择合理率、术前0.5~1 h用药时机合格率均高于对照组,差异有统计学意义(P<0.05)。观察组患者手外科、足踝科、关节外科、脊柱科、创伤骨科的抗菌药物预防使用率均低于对照组,差异有统计学意义(P<0.05)。观察组患者的术后用药时间短于对照组,疗程<24 h率高于对照组,差异有统计学意义(P<0.05)。结论PDCA循环管理有效推动了骨科Ⅰ类切口围手术期抗菌药物的规范化管理。
Objective To evaluate the application effect of PDCA cycle management in the prevention of antibiotics in orthopedic patients undergoing classⅠincision surgery.Methods A retrospective analysis was performed on 1399 patients undergoing classⅠincision surgery discharged from the Xuzhou Renren Hospital from January to March 2022 as the control group,and 984 patients discharged from April to June 2022 as the observation group.The control group received routine management,and the observation group received PDCA cycle management.The prophylactic application of antibiotics in perioperative period,the prophylactic utilization rate of antibiotics in key orthopaedic specialties,the course of prophylactic antibiotics in perioperative period and the infection rate were compared between the two groups.Results The utilization rate of antimicrobial prophylaxis in the observation group was lower than that in the control group,and the qualified rate of doctors'knowledge of antibiotics,rational rate of variety selection,and the qualified rate of 0.5 to 1 hour before operation were higher than those in the control group,the differences were statistically significant(P<0.05).The utilization rate of antimicrobial prophylaxis in the department of hand surgery,foot and ankle surgery,joint surgery,spine department,trauma and orthopedics in the observation group were lower than those in the control group,the differences were statistically significant(P<0.05).The postoperative medication time of the observation group was shorter than that of the control group,and the rate of the course of treatment<24 h was higher than that of the control group,the differences were statistically significant(P<0.05).Conclusion PDCA cycle management can effectively promote the standardized management of antibiotics in the perioperative period of typeⅠincision.
