摘要
目的研究维持性血液透析(HD)肾功能不全患者的血清分泌型卷曲相关蛋白5(Sfrp5)、Klotho蛋白水平变化及其与钙磷代谢的相关性。方法回顾性选取2021年1月至2022年1月入院的60例维持性HD肾功能不全患者与60名体检健康者,分别设定为研究组与对照组。比较两组血清Sfrp5、Klotho水平,差异Pearson方法评估血清Sfrp5、Klotho水平与全段甲状旁腺激素(iPTH)、钙磷乘积(Ca×P)的相关性,同时比较60例维持性HD肾功能不全患者中出现颈动脉硬化患者与未出现颈动脉硬化患者的血清Sfrp5、Klotho水平,并比较不同腹主动脉钙化程度患者的血清Sfrp5、Klotho水平。结果研究组患者的血清Sfrp5、Klotho水平为(38.91±10.34)μg/L、(1.18±0.61)ng/L,明显低于对照组[(75.75±17.52)μg/L、(3.02±0.96)ng/L],差异均有统计学意义(P<0.05)。血清Sfrp5、Klotho水平与iPTH、Ca×P呈负相关(r=-0.520、-0.467;-0.502、-0.474,P<0.05)。研究组颈动脉硬化患者的血清Sfrp5、Klotho水平为(35.86±10.17)μg/L、(0.95±0.38)ng/L,明显低于颈动脉未硬化患者[(49.04±9.25)μg/L、(1.84±0.51)ng/L],差异均有统计学意义(P<0.05)。研究组不同钙化程度患者的血清Sfrp5、Klotho水平差异均有统计学意义(P<0.05),随血清Sfrp5、Klotho水平降低钙化程度明显加重。结论维持性HD肾功能不全患者的血清Sfrp5、Klotho水平较健康人群明显更低,其血清Sfrp5、Klotho水平与钙磷代谢密切相关。
Objective To study the correlation of serum secreted frizzled-related protein 5(Sfrp5)and Klotho protein levels in patients with renal insufficiency on maintenance hemodialysis(HD)and their relationship with calcium and phosphorus metabolism.Methods A retrospective study of 60 patients with maintained HD renal insufficiency and 60 healthy individuals who were admitted to Chongqing Fifth People’s Hospital from January 2021 to January 2022 were selected as the study group and the control group,respectively.The levels of serum Sfrp5 and Klotho were compared between the two groups,and the Pearson method was used to evaluate the correlation of serum Sfrp5 and Klotho levels with whole-segment parathyroid hormone(iPTH)and calcium-phosphorus product(Ca×P).Serum levels of Sfrp5 and Klotho in patients with carotid atherosclerosis and those without carotid atherosclerosis,and compared the levels of serum Sfrp5 and Klotho in patients with different degrees of abdominal aortic calcification.Results The levels of serum Sfrp5 and Klotho in the study group were(38.91±10.34)μg/L,(1.18±0.61)ng/L,which were significantly lower than those in the control group[(75.75±17.52)μg/L,(3.02±0.96)ng/L],the differences were statistically significant(P<0.05).Serum Sfrp5 and Klotho levels were negatively correlated with iPTH and Ca×P(r=-0.520,-0.467;-0.502,-0.474,P<0.05).The serum Sfrp5 and Klotho levels of carotid atherosclerosis patients in the study group were(35.86±10.17)μg/L,(0.95±0.38)ng/L,which were significantly lower than those in patients without carotid atherosclerosis[(49.04±9.25)μg/L,(1.84±0.51)ng/L],the differences were statistically significant(P<0.05).The levels of serum Sfrp5 and Klotho in patients with different degrees of calcification in the study group were statistically significant(P<0.05),and the degree of calcification increased significantly with the decrease of serum Sfrp5 and Klotho levels.Conclusion The serum levels of Sfrp5 and Klotho in patients with maintenance HD renal insufficiency are significantly lower than those in healthy people,and the serum levels of Sfrp5 and Klotho are closely related to calcium and phosphorus metabolism.
