摘要
目的分析2017—2021年吉林省单胎儿与双胎儿先天性心脏病(CHD)的发生情况,为更深层次了解双胎CHD问题提供参考。方法回顾性分析2017—2021年吉林省出生缺陷上报系统发现的CHD患儿,分析单胎与双胎CHD的发生情况及临床特征。采用SPSS软件进行统计学分析,组间比较采用χ^(2)检验。结果2017—2021年吉林省CHD发生率为3.33‰,单胎和双胎CHD发生率分别为3.04‰和27.04‰,单胎与双胎CHD发生率差异有统计学意义(χ^(2)=1236.06,P=0.000)。CHD类型中单纯心血管间交通发生率最高(2.49‰),其次依次为合并畸形(0.52‰)、复杂畸形(0.13‰)、心脏瓣膜畸形(0.10‰)等。双胎CHD早产发生率和低出生体质量发生率均高于单胎,产前确诊率低于单胎(χ^(2)=69.36,P=0.000;χ^(2)=71.05,P=0.000;χ^(2)=21.89,P=0.000)。结论应重视双胎孕产妇的产前筛查、产前诊断管理,加强对双胎CHD胎儿孕产妇产期及胎儿发育的孕期监测,提高医疗保健专业人员的超声心动检查等技能,制定和完善有针对性的双胎CHD预防对策及干预措施。
Objective The incidence of CHD in singleton and twins in Jilin province from 2017 to 2021 was retrospectively analyzed,which provides a reference for a deeper understanding of the problem of twins CHD.Methods Retrospective analysis of children with CHD detected by the birth defect reporting system in Jilin province from 2017-2021,and analysis of the occurrence and clinical characteristics of children with CHD in singleton and twin pregnancies.SPSS software was used for statistical analysis,and χ^(2) test was used for comparison between groups.Results From 2017 to 2021,the incidence of neonatal CHD in Jilin province was 3.33‰,the incidence of CHD in singleton infants was 3.04‰,and the incidence of CHD in twins was 27.04‰.The difference between the incidence of CHD in singleton and twins was statistically significant(χ^(2)=1236.06,P=0.000).Among the types of CHD,the incidence of simple cardiovascular traffic was the highest(2.49‰),followed by combined malformations(0.52‰),complex malformations(0.13‰),and heart valve malformations(0.10‰),etc.The incidence of preterm birth and low birth weight in twins with CHD were higher than that of singletons,and the prenatal diagnosis rate was lower than that of singletons(χ^(2)=69.36,P=0.000;χ^(2)=71.05,P=0.000;χ^(2)=21.89,P=0.000).Conclusion We should pay attention to the prenatal screening and prenatal diagnosis management of twin pregnancies,strengthen the monitoring of premature delivery and fetal development of maternal with twin CHD fetuses,improving the skills of healthcare professionals in echocardiography and other skills,and developing and improving targeted preventive countermeasures and interventions for twins with CHD.
