局部振动联合康复训练在卒中后偏瘫下肢功能障碍患者中的应用效果 被引量:1

摘要 目的:观察局部振动联合康复训练在卒中后偏瘫下肢功能障碍患者中的应用效果。方法:回顾性分析2019年1月至2020年12月大连港医院收治的120例卒中后偏瘫下肢功能障碍患者的临床资料,按干预措施不同将其分为对照组与观察组各60例。对照组给予康复训练,观察组在对照组基础上联合局部振动,比较两组干预前后下肢痉挛程度[改良Ashworth痉挛量表(MAS)]、生命质量[简明健康调查量表(SF-36)]、日常生活活动能力[改良Barthel指数(MBI)]、平衡功能[Berg平衡量表(BBS)]和下肢运动功能评分。结果:干预1、3、6个月后,两组MAS评分均低于干预前,且观察组低于对照组,差异有统计学意义(P<0.05);两组生理职能、情感职能、躯体疼痛、总体健康、生理功能、社会功能、精力、精神健康等SF-36各维度评分均高于干预前,且观察组高于对照组,差异有统计学意义(P<0.05);两组MBI评分均高于干预前,且观察组高于对照组,差异有统计学意义(P<0.05);两组BBS评分均高于干预前,且观察组高于对照组,差异有统计学意义(P<0.05);两组下肢运动功能评分均高于干预前,且观察组高于对照组,差异有统计学意义(P<0.05)。结论:局部振动联合康复训练应用于卒中后偏瘫下肢功能障碍患者可降低MAS评分,提高SF-36、MBI、BBS评分和下肢运动功能评分,其效果优于单纯康复训练。 Objective: To observe application effects of local vibration combined with rehabilitation training in patients with hemiplegic lower limb dysfunction after stroke. Methods: The clinical data of 120 patients with hemiplegia lower limb dysfunction after stroke admitted to Dalian Port Hospital from January 2019 to December 2020 were retrospectively analyzed. They were divided into control group and observation group according to different intervention measures, 60 cases in each group. The control group was given rehabilitation training, while the observation group was combined with local vibration on the basis of that of the control group. The degree of lower limb spasm [modified Ashworth scale(MAS)],the quality of life [short form health survey scale(SF-36)], the activities of daily living [modified Barthel index(MBI)], the balance function [Berg balance scale(BBS)], and the lower limb motor function score were compared between the two groups before and after the intervention. Results:After 1, 3 and 6 months of intervention, the MAS scores of the two groups were lower than those before the intervention, those in the observation group were lower than those in the control group, and the differences were statistically significant(P<0.05). The SF-36 scores of physiological function, emotional function, physical pain, general health, physiological function, social function, vitality and mental health in the two groups were higher than those before the intervention, those in the observation group were higher than those in the control group, and the differences were statistically significant(P<0.05). The MBI scores of the two groups were higher than those before the intervention, that of the observation group was higher than that of the control group, and the differences were statistically significant(P<0.05). The BBS scores of the two groups were higher than those before the intervention, that in the observation group was higher than that in the control group, and the differences were statistically significant(P<0.05). Further, the lower limb motor function scores of the two groups were higher than those before the intervention, that in the observation group was higher than that in the control group, and the differences were statistically significant(P<0.05). Conclusions: Local vibration combined with rehabilitation training can reduce the MAS scores, improve the SF-36, MBI, BBS scores and the lower limb motor function score in the patients with hemiplegic lower limb dysfunction after stroke. Moreover, it is superior to single rehabilitation training.
