噻托溴铵吸入联合肺康复训练治疗慢性阻塞性肺疾病患者的效果 被引量:4

Effect of Tiotropium Bromide Inhalation Combined with Pulmonary Rehabilitation Training in the Treatment of Patients with Chronic Obstructive Pulmonary Disease

摘要 目的:探讨噻托溴铵吸入联合肺康复训练治疗慢性阻塞性肺疾病(COPD)患者的效果。方法:将2020年1—12月北京市石景山医院内科收治的80例COPD患者随机分为对照组(n=40,常规治疗)和研究组(n=40,噻托溴铵吸入联合肺康复训练治疗)。对比治疗前后两组肺功能、炎症因子、血气分析及COPD评估测试(CAT)评分。结果:两组治疗前肺功能、炎症因子、血气分析及CAT评分比较,差异均无统计学意义(P>0.05);研究组治疗后呼气流量峰值(PEF)、第1秒用力呼气容积(FEV_(1))、用力肺活量(FVC)、FEV_(1)/FVC%、白细胞介素-17(IL-17)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、动脉血二氧化碳分压(PaCO_(2))、血氧分压(PaO_(2))及情绪、精力、睡眠、胸闷、咳痰、咳嗽、日常运动及运动耐力评分均优于对照组,差异均有统计学意义(P<0.05)。结论:噻托溴铵吸入联合肺康复训练可有效改善COPD患者临床症状,恢复肺功能,抑制炎症反应。 Objective: To investigate the effect of Tiotropium Bromide inhalation combined with pulmonary rehabilitation training in the treatment of patients with chronic obstructive pulmonary disease(COPD). Method: From January to December 2020, 80 patients with COPD who treated in the Internal Medicine Department of Beijing Shijingshan Hospital were randomly divided into the control group(n=40 cases, conventional treatment) and the study group(n=40, Tiotropium Bromide inhalation combined with pulmonary rehabilitation training). The pulmonary function, inflammatory factors, blood gas analysis and COPD assessment test scores were compared between two groups before and after treatment. Result: There were no significant differences in lung function, inflammatory factors, blood gas analysis and CAT score between two groups before treatment(P>0.05). The peak expiratory flow(PEF), forced expiratory volume in the first second(FEV_(1)), forced vital capacity(FVC), FEV_(1)/FVC%, interleukin-17(IL-17), interleukin-6(IL-6), tumor necrosis factor-α(TNF-α),arterial partial pressure of carbon dioxide(PaCO_(2)), oxygen fraction pressure(PaO_(2)) and scores of mood, energy, sleep, chest tightness,sputum, cough, daily exercise and exercise endurance in the study group after treatment were better than those in the control group, the differences were statistically significant(P<0.05). Conclusion: Tiotropium Bromide inhalation combined with pulmonary rehabilitation training can effectively improve the clinical symptoms of COPD patients, recover lung function symptoms and inhibit inflammation.
