摘要
目的:分析纤维支气管镜(纤支镜)治疗呼吸重症监护室(RICU)肺部感染的效果。方法:选取2021年3月—2022年7月荆州市胸科医院收治的84例RICU肺部感染患者。根据随机数表法将其分为对照组及观察组,各42例。对照组给予常规治疗,观察组在常规治疗基础上给予纤支镜吸痰灌洗治疗。比较两组临床疗效,临床症状改善情况,治疗前后炎症因子、血气指标。结果:观察组治疗总有效率为95.24%,高于对照组的78.57%(P<0.05)。观察组抗菌药物使用时间短于对照组,肺部啰音消失时间、体温恢复时间、肺部病变吸收>75%时间及血象恢复时间均早于对照组(P<0.05)。治疗后,两组降钙素原(PCT)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平均较治疗前低,观察组PCT、IL-6、TNF-α水平均低于对照组(P<0.05)。治疗后,两组pH值、PaO_(2)均较治疗前高,PaCO_(2)较治疗前低,且观察组pH值、PaO_(2)均高于对照组,PaCO_(2)低于对照组(P<0.05)。结论:对RICU肺部感染患者应用纤支镜吸痰灌洗治疗可有效提高治疗效果,尽快缓解患者临床症状,促进气道分泌物的清除,改善患者血气指标,提高通气功能,同时降低炎症因子水平,改善患者预后。
Objective:To analyze the effect of fiber bronchoscope sputum aspiration and lavage in the treatment of pulmonary infection in respiratory intensive care unit(RICU).Method:A total of 84 patients with pulmonary infection in RICU admitted to Jingzhou Chest Hospital from March 2021 to July 2022 were selected.According to the random number table method,they were divided into control group and observation group,42 cases in each group.The control group was given routine treatment,and the observation group was given fiber bronchoscope sputum aspiration and lavage treatment on the basis of routine treatment.The clinical efficacy,improvement condition of clinical symptoms,inflammatory factors and blood gas indexes before and after treatment were compared between the two groups.Result:The treatment total effective rate of the observation group was 95.24%,which was higher than 78.57%of the control group(P<0.05).The use time of antibiotics in the observation group was shorter than that in the control group,and the disappearance time of lung rale,the recovery time of body temperature,the absorption of lung lesions>75%time and the recovery time of blood picture in the observation group were earlier than those in the control group(P<0.05).After treatment,the levels of procalcitonin(PCT),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)in the two groups were lower than those before treatment,and the levels of PCT,IL-6 and TNF-αin the observation group were lower than those in the control group(P<0.05).After treatment,the pH value and PaO_(2) of the two groups were higher than those before treatment,and the PaCO_(2) was lower than that before treatment,and the pH value and PaO_(2) of the observation group were higher than those of the control group,and the PaCO_(2) was lower than that of the control group(P<0.05).Conclusion:The application of fiber bronchoscope sputum aspiration and lavage treatment in RICU patients with pulmonary infection can effectively improve the therapeutic effect,relieve the clinical symptoms of the patients as soon as possible,promote the clearance of airway secretions,improve the blood gas indexes of the patients,improve the ventilation function,and reduce the level of inflammatory factors at the same time,improve the prognosis of patients.
