集束化干预在重症监护病房患者中的应用效果

摘要 目的:探究集束化干预在重症监护病房患者中的应用效果。方法:选取2020年4月—2022年2月北京市垂杨柳医院收治的110例重症监护病房患者。根据不同就诊时间将其分为对照组(2020年4月—2021年1月)(64例)和观察组(2021年2月—2022年2月)(46例)。对照组实施常规干预,观察组在对照组基础上增加集束化干预。比较两组多重耐药菌感染发生率、清洁消毒效果、手卫生依从率、平均住院费用及住院时间。结果:观察组多重耐药菌感染发生率低于对照组(χ^(2)=3.990,P=0.046)。观察组呼吸机、监护仪、床头桌、床栏、病历夹清洁消毒效果合格率高于对照组(P<0.05)。观察组接触患者前、清洁/无菌操作前、体液暴露风险后、接触患者后、接触患者周围环境后手卫生依从率均高于对照组(P<0.05)。观察组平均住院费用少于对照组,住院时间短于对照组(P<0.05)。结论:集束化干预在重症监护病房中的应用可降低多重耐药菌感染发生率,提高接触物表面或者仪器清洁消毒效果,同时也提高了手卫生依从率,缩短住院时间的同时减少了住院费用。 Objective:To explore application effect of cluster intervention in the patients in intensive care unit.Method:A total of 110 patients in intensive care unit admitted to Weeping Willow Hospital of Beijing from April 2020 to February 2022 were selected.They were divided into control group(from April 2020 to January 2021)(64 cases)and observation group(from February 2021 to February 2022)(46 cases)according to different treatment time.The control group received routine intervention,and the observation group was supplemented with cluster intervention on the basis of control group.The incidence of multidrug-resistant bacteria infection,cleaning and disinfection effect,hand hygiene compliance rate,average hospitalization cost and hospital stay were compared between the two groups.Result:The incidence of multidrug-resistant bacteria in the observation group was lower than that in the control group(χ^(2)=3.990,P=0.046).The qualified rates of ventilator,monitor,bedside table,bed rail,caller and medical record clip cleaning and disinfection in observation group were higher than those in control group(P<0.05).The hand hygiene compliance rate of observation group was higher than that of control group in before contact with patients,before cleaning/aseptic operation,after exposure to body fluid risk,after contact with patients and after contact with patients'surroundings(P<0.05).The average hospitalization cost of the observation group was less than that of the control group,and the hospital time was shorter than that of the control group(P<0.05).Conclusion:The application of the cluster intervention in the intensive care unit can reduce the incidence of multi-drug resistant bacteria infection,improve the cleaning and disinfection effect of the contact surface or instrument,and also improve the compliance rate of hand hygiene,shorten the hospitalization time and reduce the hospitalization cost.
