摘要
目的:系统评价渗透树脂治疗正畸后白垩斑的美学效果以及安全性。方法:计算机检索CNKI、万方数据库、维普数据库、PubMed、Cochrane Library、EMbase、CBM,同时追溯纳入文献和相关综述的参考文献,收集关于渗透树脂治疗正畸后白垩斑的美学效果随机对照试验(Random control trial,RCT),对照组予以微研磨方式治疗,实验组予以渗透树脂治疗。设置检索时间为2016年1月-2020年5月,由两名研究员根据纳入与排除标准筛选文献、提取资料评价纳入研究的偏倚风险,采取RevMan 5.3进行Meta分析。主要观察指标:治疗有效率;次要观察指标:不良反应发生率及发表偏倚。结果:实验组有效率为89.49%,显著高于对照组的65.28%,相对危险度(Riskratio,RR)及95%的置信区间(Confidenceinterval, CI)为4.62(3.16~6.75)(Z=7.91, P<0.00 001)。实验组不良反应发生率为2.20%(9/409),不良反应主要有龋齿7例,脱矿变色2例;对照组不良反应发生率为9.78%(40/409),不良反应主要有龋齿27例,脱矿变色13例,合并RR及95%CI为0.22(0.11~0.45)(Z=4.15, P<0.00001)。结论:渗透树脂用于治疗正畸后白垩斑患者可提高美学效果,不良反应发生率低,安全性更高。
Objective To evaluate the aesthetic effect and safety of resin infiltration treatment. Methods Databases such as CNKI, Wanfang database, Vip database, PubMed, EMbase, CBM were run. References in the literature and related reviews and randomized controlled trials(RCT) on the aesthetic effects of resin infiltration treatment were also collected. The control group was micro-milled, while the experimental group was treated with resin infiltration. The retrieval time was set from January 2016to May 2020. Two researchers screened the literature according to the inclusion and exclusion criteria. And data were extracted to evaluate the risk of bias in the included studies, and RevMan5. 3 was used for meta-analysis. Main outcome indicators were treatment efficiency. Secondary outcome indicators were incidence of adverse reactions and publication bias. Results The effective rate in the experimental group was 89. 49%, which was significantly higher than that of control group(65.28%).The combined RR and 95%CI were 4.62(3.16~6.75)(Z =7.91,P <0.00 001).The incidence rate of adverse reactions in the experimental group was 2.20%(9/409).7 cases were mainly caries, 2 cases were demineralized and discolored.And the incidence of adverse reactions in the control group was 9.78%(40/409). 27 cases of caries, and 13 cases of demineralization and discoloration, combined RR and 95% CI were 0. 22(0.11~0.45)(Z=4.15, P<0.00 001). Conclusion Resin infiltration treatment can be used to treat chalk spots and to improve aesthetic effects, with a low incidence of adverse reactions and higher safety.
