X线平片、CT及MRI对踝关节运动损伤的诊断价值

摘要 目的:比较X线平片、计算机断层扫描(CT)及磁共振成像(MRI)对踝关节运动损伤的诊断价值。方法:选取2019年8月至2022年8月在徐州市铜山区中医院就诊的120例疑似踝关节运动损伤者作为研究对象,均开展X线平片检查、CT检查、MRI检查,比较三种检查方法的诊断效果。结果:MRI检查的灵敏度、特异度、准确度、阳性预测值及阴性预测值均比X线平片、CT检查更高,且CT检查各项指标均高于X线平片,差异均具有统计学意义(P <0.05)。CT检查、MRI检查的诊断结果与临床最终诊断结果之间的一致性高(κ=0.742、0.893,P均<0.05),而X线平片的诊断结果与临床最终诊断结果之间的一致性为中等(κ=0.521,P <0.05)。对于韧带肌腱损伤、关节软骨损伤,MRI检出率明显高于CT和X线平片,差异均具有统计学意义(P <0.05),CT与X线平片比较,差异无统计学意义(P> 0.05);在骨折的检出率上,CT高于MRI和X线平片,差异均具有统计学意义(P <0.05),而MRI与X线平片比较,差异无统计学意义(P> 0.05)。结论:对于韧带肌腱损伤、关节软骨损伤,MRI检查具有显著优势,对于骨折的诊断CT优于MRI和X线平片。 Objective To compare the diagnostic value of X-ray plain film, computed tomography(CT) and magnetic resonance imaging(MRI) in ankle joint sports injury. Methods A total of 120 patients suspected of ankle joint sports injury treated in Xuzhou Tongshan District Hospital of Traditional Chinese Medicine from August 2019 to August 2022 were selected as the research objects. X-ray plain film, CT examination and MRI examination were performed to compare the diagnostic effects of the three examination methods. Results The sensitivity, specificity, accuracy, positive predictive value and negative predictive value of MRI examination were higher than those of X-ray plain film and CT examination, and all the indicators of CT examination were higher than those of X-ray plain film, the differences were statistically significant(P < 0.05). The consistency between CT examination and MRI examination diagnosis results and clinical final diagnosis were high(κ = 0.742, 0.893, P < 0.05), while the consistency between the X-ray plain film diagnosis results and clinical final diagnosis were moderate(κ = 0.521, P < 0.05). For ligament tendon injury and articular cartilage injury, the detection rate of MRI was significantly higher than that of CT and X-ray plain film, the differences were statistically significant(P < 0.05), but there was no statistical significance between CT and plain X-ray film(P > 0.05). In terms of fracture detection rate, CT was higher than MRI and plain X-ray, the differences were statistically significant(P < 0.05), but there was no statistically significant difference between MRI and X-ray plain film(P > 0.05). Conclusion For ligament tendon injury and articular cartilage injury, MRI examination has significant advantages, and CT is superior to MRI and X-ray plain film in the diagnosis of fracture.
