摘要
目的:探讨实时三维经食管超声心动图在人工瓣瓣膜梗阻诊断及原因分析中的应用价值。方法:回顾性分析61例存在瓣膜梗阻病例其术前经胸超声心动图及实时三维经食管超声心动图的梗阻诊断符合情况,并进一步分析其术前原因探查的诊断符合率。结果:(1)术前实时三维经食管超声心动图诊断人工瓣瓣膜梗阻总体符合率高于经胸超声心动图,两种诊断方法存在统计学差异(P<0.05),其中两者在诊断机械瓣瓣膜梗阻符合率存在统计学差异(P<0.05),在诊断生物瓣瓣膜梗阻符合率不存在统计学差异(P>0.05);(2)术前实时三维经食管超声心动图对人工瓣瓣膜梗阻原因诊断符合率高于经胸超声心动图,两种诊断方法之间存在统计学差异(P<0.05),其中两者诊断血管翳存在统计学差异(P<0.05)。结论:实时三维经食管超声心动图相对经胸超声心动图对人工瓣瓣膜梗阻的诊断准确率更高,并对梗阻原因提供更多有价值信息,是重要的评估手段。
Objective:To explore the value of real-time three-dimensional transesophageal echocardiography in diagnosis and cause analysis of prosthetic valve obstruction.Methods:Retrospective analysis of the 61 cases whose preoperative diagnosis of obstruction by transthoracic echocardiography and real-time three-dimensional transesophageal echocardiography,and further analyzed the cause accuracy rate of them.Results:(1)The overall accuracy rate of preoperative real-time three-dimensional transesophageal echocardiography in the diagnosis of prosthetic valve obstruction was higher than that of transthoracic echocardiography,and there was a statistical difference between the two diagnostic methods(P<0.05).There was a statistical difference in the accuracy rate of Mechanical prosthetic obstruction between the two diagnostic methods(P<0.05).There was no statistical difference in the accuracy rate of diagnosis of biological prosthesis valve obstruction(P>0.05).(2)The diagnostic accuracy rate of preoperative real-time hree-dimensional transesophageal echocardiography for prosthetic valve obstruction was higher than that of transthoracic echocardiography,and there was statistical difference between the two diagnostic methods(P<0.05),and there was statistical difference between the two diagnostic methods for pannus(P<0.05).Conclusion:Real-time three-dimensional transesophageal echocardiography is more accurate than transthoracic echocardiography in the diagnosis of prosthetic valve obstruction,and provides more valuable information about the cause of obstruction,which is an important evaluation method.
