PCT、CHE及CRP与急性有机磷中毒患者呼吸衰竭的关系 被引量:4

Relationship between PCT,CHE and CRP and respiratory failure in patients with acute organophosphorus poisoning

摘要 目的 分析降钙素原(PCT)、血清胆碱酯酶(CHE)及C-反应蛋白(CRP)与急性有机磷中毒(AOPP)患者呼吸衰竭的关系。方法 选取太和县人民医院2018年7月至2022年4月收治的AOPP患者102例,根据有无呼吸衰竭发生分为无呼衰组33例(无呼吸衰竭)和呼衰组69例(呼吸衰竭)。分析两组入院即刻、治疗后1 d、治疗后3 d PCT、CHE及CRP水平,对比不同中毒程度AOPP的PCT、CHE及CRP水平及呼吸衰竭发生率。结果 两组在入院即刻、治疗后1 d、治疗后3 d PCT水平呈上升,且呼衰组在各时间段PCT均高于无呼衰组,差异均有统计学意义(P<0.05)。两组在入院即刻、治疗后1 d、治疗后3 d CHE水平呈上升,且呼衰组在各时间段CHE均低于无呼衰组,差异均有统计学意义(P<0.05)。两组在入院即刻、治疗后1 d、治疗后3 d CRP水平呈上升,且呼衰组各时间段CRP水平高于无呼衰组,差异均有统计学意义(P<0.05)。轻度中毒AOPP患者32例,中度中毒AOPP患者14例,重度中毒AOPP患者56例。AOPP患者PCT、CHE及CRP水平及呼吸衰竭发生率:重度>轻度>中度,差异均有统计学意义(P<0.05)。结论 PCT、CHE及CRP与急性有机磷中毒患者呼吸衰竭存在密切联系,对其后续治疗和预后有一定指导意义。 Objective To analyze the relationship between procalcitonin(PCT),serum cholinesterase(CHE)and C-reactive protein(CRP)and respiratory failure in patients with acute organophosphorus poisoning(AOPP). Methods 102 patients with AOPP who were admitted to our hospital from July 2018 to April2022 were selected,and according to the occurrence of failure,they were divided into the non-respiratory failure group of 33 cases(without respiratory failure)and the respiratory failure group of 69 cases(respiratory failure). The levels of PCT,CHE and CRP in different time periods of the two groups were analyzed,and the levels of PCT,CHE and CRP of AOPP with different degrees of poisoning and the incidence of respiratory failure were compared. Results The levels of PCT in the two groups after admission,1 d after treatment,and 3 d after treatment increased immediately,and the PCT in the respiratory failure group was higher than that in the nonrespiratory failure group at each time period(P<0.05). The levels of CHE in the two groups after admission,1d after treatment,and 3 d after treatment increased immediately,and the CHE in the respiratory failure group was lower than that in the non-respiratory failure group at each time period(P<0.05). The CRP levels in the two groups increased immediately after admission,1 day after treatment and 3 days after treatment,and the CRP levels in the respiratory failure group were higher than those in the non-respiratory failure group at all time points,with statistical significancep(P<0.05). There were 32 patients with mild poisoning AOPP,14 patients with moderate poisoning AOPP,and 56 patients with severe poisoning AOPP. The levels of PCT,CHE and CRP and the incidence of respiratory failure in AOPP patients were severe > mild > moderate,and the difference was statistically significant(P<0.05). Conclusion PCT,CHE and CRP are closely related to respiratory failure in patients with acute organophosphorus poisoning,which have certain guiding significance for their follow-up treatment and prognosis.
