GFR联合血清Cys C、β2-MG对不同程度肾积水患者的诊断价值 被引量:3

Diagnostic value of GFR combined with serum Cys C and β2-MG in patients with different degrees of hydronephrosis

摘要 目的 探究肾小球滤过率(GFR)联合血清胱抑素C(Cys C)、β2微球蛋白(β2-MG)对不同程度肾积水患者的诊断价值。方法 选取2020年1月至2020年11月珠海市中西医结合医院泌尿外科收治的肾积水患者118例为研究对象,根据Arger分级标准分为轻度肾积水组(n=48)、中度肾积水组(n=37)与重度肾积水组(n=33)。采用99mTc-DTPA肾动态显像、ELISA法与全自动生化检测仪检测并对比三组患者GFR、血清Cys C、β2-MG水平,并绘制ROC曲线分析上述指标单一及联合检测对肾积水患者的诊断价值。结果 三组GFR水平:轻度肾积水组>中度肾积水组>重度肾积水组,Cys C、β2-MG水平:重度肾积水组>中度肾积水组>轻度肾积水组,差异均具有统计学意义(F=8.377、13.301、354.459,P<0.05)。Pearson相关性分析结果显示,血清Cys C、β2-MG水平与GFR呈负相关(P<0.05)。ROC曲线显示,GFR、Cys C、β2-MG三者联合检测时,对肾积水的AUC为0.904,敏感性、特异性分别为91.7%、89.6%,优于单一检测(P<0.05)。结论 在常规检测GFR的基础上联合检测CysC、β2-MG可提高不同程度肾积水诊断的敏感性与准确性,可为肾积水的临床诊疗提供参考依据。 Objective To explore the diagnostic value of glomerular filtration rate(GFR)combined with serum cystatin C(Cys C)and β2microglobulin(β2-MG)in patients with different degrees of hydronephrosis. Methods From January 2020 to November 2020,118 patients with hydronephrosis who were admitted to the Department of Urology,Zhuhai Integrated Traditional Chinese and Western Medicine Hospital were selected as the research objects. According to the Arger classification,they were divided into the mild hydronephrosis group(n=48),the moderate Hydronephrosis group(n=37)and the severe hydronephrosis group(n=33).99mTc-DTPA renal dynamic imaging,ELISA method and automatic biochemical detector were used to detect and compare the levels of GFR,serum Cys C and β2-MG in three groups of patients and draw ROC curve to analyze the effects of single and combined detection of the above indicators on hydronephrosis diagnostic value. Results The results showed that GFR level:mild hydronephrosis group > moderate hydronephrosis group > severe hydronephrosis group;Cys C and β2-MG levels:severe hydronephrosis group > moderate hydronephrosis group > mild hydronephrosis. In the hydronephrosis group,the difference was statistically significant(F=8.377,13.301,354.459,P<0.05). The results of Pearson correlation analysis showed that serum Cys C and β2-MG levels were negatively correlated with GFR(P<0.05). The ROC curve showed that when GFR,Cys C,and β2-MG were combined to detect the three,the AUC for hydronephrosis was 0.904,and the sensitivity and specificity were 91.7% and 89.6%,respectively,which were better than single detection(P<0.05).Conclusion Combined detection of CysC and β2-MG on the basis of routine detection of GFR can improve the sensitivity and accuracy of the diagnosis of different degrees of hydronephrosis and can provide a reference for the clinical diagnosis and treatment of hydronephrosis.
