摘要
目的 探究血清总胆红素、网织红细胞联合血常规检测对新生儿溶血病早期诊断的价值。方法 分析2019年7月至2020年6月阜阳市人民医院205例母婴血型不合新生儿的临床资料,将发生溶血病的新生儿归为发生组(n=61),未发生溶血病的新生儿归为未发生组(n=144)。对比两组新生儿的血清总胆红素、网织红细胞水平及血常规检测情况,并采用ROC曲线分析,血清总胆红素、网织红细胞联合血常规检测对新生儿溶血病的诊断价值。结果 发生组的血清总胆红素及网织红细胞水平明显高于未发生组,差异有统计学意义(t=12.492,5.875;P均<0.05);发生组的WBC水平明显高于未发生组,RBC及Hb水平明显低于未发生组,差异有统计学意义(t=4.334,3.079,4.168;P均<0.05);两组的PLT及RDW水平比较,差异无统计学意义(P>0.05)。ROC曲线分析结果显示,血清总胆红素、网织红细胞、WBC、RBC、Hb预测新生儿溶血病的曲线下面积分别为0.901、0.808、0.681、0.619、0.674;血清总胆红素、网织红细胞、联合血常规指标(WBC、RBC、Hb)预测新生儿溶血病的面积为0.934。结论 血清总胆红素、网织红细胞联合血常规检测可准确的对新生儿溶血病进行诊断。
Objective To explore the value of serum total bilirubin,reticulocyte combined with routine blood test in early diagnosis of hemolytic disease of newborn. Methods The clinical data of 205newborns with maternal-infant blood group incompatibility in Fuyang People’s Hospital from July 2019 to June2020 were retrospectively analyzed. Newborns with hemolytic disease were classified as the occurrence group(n=61)and those without hemolytic disease were classified as the non-occurrence group(n=144). The levels of serum total bilirubin,reticulocyte and routine blood tests were compared between the two groups,and ROC curve analysis was used. The combined blood tests of serum total bilirubin and reticulocyte were used to hemolyze the newborn. Results The serum total bilirubin and reticulocyte levels in the occurrence group were significantly higher than those in the non-occurrence group,and the difference was statistically significant(t=12.492,5.875;P<0.05). The WBC level in the occurrence group was significantly higher than that in the nonoccurrence group,and the RBC and Hb levels were significantly lower than that in the non-occurrence group,with statistical significance(t=4.334,3.079,4.168;P<0.05). There was no significant difference in PLT and RDW levels between the two groups(P>0.05). The ROC curve analysis showed that the areas under the curve of serum total bilirubin,reticulocyte,WBC,RBC and Hb were 0.901,0.808,0.681,0.619 and 0.674,respectively. The area predicted by serum total bilirubin,reticulocyte and combined blood routine indexes (WBC,RBC and Hb)is 0.934. Conclusion Serum total bilirubin,reticulocyte combined with routine blood test can accurately diagnose newborn hemolytic disease.
