摘要
目的:对2型糖尿病(T2DM)患者发生周围神经病变(DPN)的相关影响因素进行探讨。方法:选取2017年~2019年间上海市虹口区8家社区卫生中心收集的6 898例T2DM患者,根据是否合并糖尿病性DPN将其分为单纯T2DM组(n=6 376)和DPN组(n=522),比较两组各项临床指标并进行分析。结果:(1)两组腹型肥胖、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、尿酸(UA)比较,差异有统计学意义(P<0.05);(2)DPN组的空腹静脉血糖(FPG)及糖化血红蛋白(HbA1c)均高于单纯T2DM组,差异有统计学意义(P<0.05);(3)相关性研究表明DPN与FPG、HbA1c、UA、腹型肥胖间存在一定的相关性(r=0.027、0.042、0.033、0.040,P<0.05);(4)Logistic多因素回归分析,结果显示腹型肥胖、LDL-C是T2DM患者发生DPN的影响因素(P<0.05)。结论:高血糖、血脂异常、腹型肥胖以及高尿酸血症为DM患者发生DPN的关键影响因素。
Objective To explore the related factors of diabetes peripheral neuropathy(DPN) in patients with type 2 diabetes mellitus(T2DM).Method 6 898 patients with T2DM collected from 8 community health centers in Hongkou District, Shanghai from 2017 to 2019 were selected and divided into simple T2DM group(n=6 376) and DPN group(n=522) according to whether they had DPN. The clinical indexes of two groups were compared and analyzed.Results(1) There was significant difference in abdominal obesity, TC, HDL-C, LDL-C and UA between the two groups(P<0.05);(2)FPG and HbA1c in DPN group were significantly higher than those in T2DM group(P<0.05);(3)The correlation study showed that DPN was correlated with FPG, HbA1c, UA and abdominal obesity(r=0.027, 0.042, 0.033, 0.040, P<0.05);(4)Logistic regression analysis showed that abdominal obesity and LDL-C were the influencing factors of DPN in T2DM patients(P<0.05).Conclusion The authors conclude that the hyperglycemia, dyslipidemia, abdominal obesity and hyperuricemia are the key influencing factors of DPN in DM patients.
