摘要
目的调查苏州市姑苏区围绝经期女性骨量丢失情况。方法以2021年3月-12月在南京医科大学附属苏州医院健康体检的40~60岁女性为研究对象,将其中符合围绝经期诊断标准的315例对象纳入此次研究。记录年龄、身高、体重、BMI,采用双能X线吸收骨密度仪检测腰椎和髋部骨密度,分析不同年龄、不同检测部位、不同BMI对骨量变化的影响。结果腰椎L1~4骨量减少发生率、骨质疏松发生率分别为25.40%(80/315)、4.76%(15/315);髋部骨量减少发生率、骨质疏松发生率分别为23.81%(75/315)、1.90%(6/315)。骨量丢失情况随年龄的增长而加重,且相同年龄段内髋部骨密度T值低于腰椎L1~4(P<0.05)。BMI过低会加剧骨量丢失,且BMI越高,骨量减少发生率越低(P<0.05)。结论年龄、检测部位和BMI均可影响围绝经期女性的骨密度。围绝经期女性骨量减少的人群庞大,应根据本地区流行病学特点展开并加强评估、宣教和管理,预防和延缓绝经后骨质疏松症的发生。
Objective To investigate the bone loss of peri-menopausal women in Gusu District of Suzhou City.Methods A total of 315 women aged 40-60 years who underwent physical examination in the Affiliated Suzhou Municipal Hospital of Nanjing Medical University from March to December 2021 were included in this study.The information of age,height,weight and BMI were recorded.The bone mineral density of lumbar spine and hip was detected by dual energy X-ray absorptiometry.The effects of different ages,different detection sites and different BMI on bone mass changes were analyzed.Results The incidence of osteopenia and osteoporosis in the lumbar spine L1-4 was 25.40%(80/315)and 4.76%(15/315),respectively.The incidence of osteopenia and osteoporosis in the hip was 23.81%(75/315)and 1.90%(6/315),respectively.Bone loss increased with age,and the T bone mineral density of the hip was lower than that of the lumbar spine L1-4 in the same age group(P<0.05).Bone loss was exacerbated by low BMI,the incidence of osteopenia decreased with the increased of BMI(P<0.05).Conclusion Age,detected sites and BMI can affect the BMD in peri-menopausal women.Due to the large number of peri-menopausal women with bone loss,we should strengthen assessment,prevention education and management to the peri-menopausal women,and prevent and delay the occurrence of postmenopausal osteoporosis.
