摘要
目的:观察单通道经皮肾镜取石术(PCNL)联合输尿管软镜碎石术(FURL)对肾结石患者应激反应及肾功能的影响。方法:选取2019年6月-2022年6月乐平市中医医院收治的82例肾结石患者,随机分为对照组、观察组,各41例。对照组进行单通道PCNL治疗,观察组进行单通道PCNL联合FURL治疗。比较两组结石清除率(SFR);于术前、术后1 d比较两组肾功能[血肌酐(Scr)、血尿素氮(BUN)]、应激反应指标[皮质醇(Cor)、超氧化物歧化酶(SOD)、丙二醛(MDA)]。结果:观察组SFR为94.05%,高于对照组的80.00%(P<0.05);术后,两组Scr、BUN较术前均显著升高,但观察组均低于对照组(P<0.05);术后,两组SOD、Cor、MDA较术前均显著升高,但观察组均低于对照组(P<0.05)。结论:单通道PCNL联合FURL治疗肾结石,可提高SFR,对肾功能影响较小,可减轻患者应激反应,利于术后恢复。
Objective:To observe the effects of single channel percutaneous nephrolithotomy(PCNL) combined with flexible ureteroscopic lithotripsy(FURL)on stress response and renal function in patients with nephrolithiasis.Method:A total of 82 patients with nephrolithiasis admitted to Leping Hospital of Traditional Chinese Medicine from June 2019 to June 2022 were selected and randomly divided into a control group and an observation group,with 41 cases in each group.The control group was treated with single channel PCNL,and the observation group was treated with single channel PCNL combined with FURL.The stone-free rate(SFR)was compared between the two groups.Renal function[serum creatinine(Scr),blood urea nitrogen(BUN)]and stress response indexes[cortisol(Cor),superoxide dismutase(SOD)and malondialdehyde(MDA)]were compared between the two groups before and 1 day after operation.Result:The SFR of the observation group was 94.05%,which was higher than 80.00%of the control group(P<0.05).After operation,Scr and BUN in both groups were significantly higher than those before operation,but those in the observation group were lower than those in the control group(P<0.05).After operation,SOD,Cor,and MDA in both groups were significantly higher than those before operation,but those in the observation group were lower than those in the control group(P<0.05).Conclusion:Single channel PCNL combined with FURL in the treatment of nephrolithiasis can improve SFR,and has little effect on renal function,which can reduce the stress response of patients and facilitate postoperative recovery.
