经鼻高流量湿化氧疗联合支气管镜下肺泡灌洗治疗慢性阻塞性肺疾病急性加重期(AECOPD)合并肺部感染疗效观察

摘要 目的研究经鼻高流量湿化氧疗(HFNC)联合支气管镜下肺泡灌洗治疗慢性阻塞性肺疾病急性加重期(AECOPD)合并肺部感染疗效.方法选择2021年3月-2022年8月江门市新会区人民医院收治确诊的慢性阻塞性肺疾病急性加重期合并肺部感染患者70例作为研究对象,随机分为观察组与对照组,各35例.对照组使用鼻导管吸氧治疗,观察组使用HFNC联合支气管镜下肺泡灌洗治疗.观察两组气管插管、非气管插管平均住院天数、白细胞(WBC)、C反应蛋白(CRP)、降钙素原(PCT)、pH、PaO_(2)、PaCO_(2)、肺功能指标,对比两组治疗效果.结果观察组非气管插管平均住院天数少于对照组,具有统计学差异(P<0.05).治疗后,观察组肺功能指标、pH、PaO_(2)水平高于对照组,PaCO_(2)水平低于对照组,具有统计学差异(P<0.05).观察组WBC、CRP、PCT水平显著低于对照组(P<0.05).观察组治疗有效率高于对照组,具有统计学差异(P<0.05).结论经鼻高流量湿化氧疗联合支气管镜下肺泡灌洗治疗慢性阻塞性肺疾病急性加重期合并肺部感染患者,可有效提升其肺功能,改善血气指标,降低WBC、CRP、PCT水平,效果显著. Objective To study the efficacy of transnasal high flow humidified oxygen therapy(HFNC)combined with bronchoscopic alveolar lavage in the treatment of acute exacerbation of chronic obstructive pulmonary disease(AECOPD)with pulmonary infection.Methods 70 patients with acute exacerbation of chronic obstructive pulmonary disease complicated with pulmonary infection who were admited to our hospital from March 2021 to August 2022 were selected and randomly divided into observation group and control group,35 cases each.The control group was treated with nasal catheter oxygen inhalation,and the observation group was treated with HFNC combined with bronchoscopic alveolar lavage.The average hospitalization days,white blood cells(WBC),C-reactive protein(CRP),procalcitonin(PCT),pH,PaO_(2),PaCO_(2) and pulmonary function indexes of the two groups were observed and compared.Results The average hospitalization days without endotracheal intubation in the observation group were lower than those in the control group(P<0.05).The pulmonary function index,pH,PaO_(2) level of the observation group were higher than those of the control group,and the PaCO_(2) level was lower than that of the control group,with statistical difference(P<0.05).The levels of WBC,CRP and PCT in the observation group were significantly lower than those in the control group(P<0.05).The effective rate of treatment in the observation group was higher than that in the control group(P<0.05).Conclusion Nasal high flow humidification oxygen therapy combined with bronchoscopic alveolar lavage can effectively improve the pulmonary function,improve blood gas indexes and reduce WBC,CRP and PCT levels in patients with COPD complicated with pulmonary infection in acuteexacerbation stage.
