摘要
目的探讨贴壁型与非贴壁型肺腺癌的CT特征。方法回顾性收集100例肺腺癌的影像和临床资料,所有患者均行CT检查,并在4周内有病理结果。根据生长方式分为贴壁组和非贴壁组,比较两组的CT征象,包括肿瘤的最大径(TMax)、实性成分最大径(Smax)、实性肿瘤比(STR)、CT值、深分叶征、毛刺征、血管异常改变、支气管异常改变、空泡征、胸膜牵拉。采用单因素方差分析上述征象在两组间是否存在差异,采用二元logistic回归将差异有统计学意义的指标纳入回归模型,并用ROC曲线评估其效能。结果贴壁组TMax、Smax、STR、CT值均大于贴壁组,差异具有统计学意义(P<0.05),非贴壁组发生血管异常以及胸膜牵拉的频率高于贴壁组(P<0.05)。将上述差异有统计学意义的指标纳入logistic模型,其AUC值均大于单个参数的AUC值(P<0.05)。结论联合多个CT特征可以有效帮助鉴别诊断贴壁型及非贴壁型肺腺癌。
Objective To investigate the CT features of lepidic and non-lepidic lung adenocarcinoma.Methods The imaging and clinical data of 100 patients with lung adenocarcinoma were collected retrospectively.All patients underwent lung CT and there were pathological results within 4 weeks.The cases were divided into lepidic group and non-lepidic group according to the pathological growth pattern.The CT signs of the two groups were compared,including tumor maximum diameter(Tmax),solid component maximum diameter(Smax),solid tumor ratio(STR),CT value,lobulation,s piculation abnormal vascular changes,abnormal bronchial changes,vacuole and pleural traction.One way ANOVA was used to analyze whether the above signs were different Statistically between the two groups.Then the indicators with statistically significant differences were brought into the regression model by binary logistic analysis and ROC curve was used to evaluate its efficacy.Results The TMax,Smax,STR and CT values of the non-lepidic group were higher than those of the lepidic group significantly(P<0.05).The frequency of vascular abnormalities and pleural traction in the nonlepidic group was higher than that in lepidic group significantly(P<0.05).The above statistically significant indicators were included in the logistic model,and the AUC value of its was higher than those of a single parameter,with the statistically significant difference(all P<0.05).Conclusion Combining multiple CT characters can improve the diagnostic efficiency in distinguish lepidic lung adenocarcinoma from non-lepidic lung adenocarcinoma effectively.
