摘要
目的:探讨术前存在根尖周疼痛的患者与根管治疗术后疼痛的相关性,分析性别,牙齿类型,颌位和术前根尖周疼痛对根管治疗术后疼痛的影响,为临床预防或减轻根管治疗术后疼痛提供帮助。方法:选取2021年1月—2022年1月合肥市口腔医院收治的133例(133颗牙齿)根管治疗患者。采用视觉模拟量表(VAS)在根管治疗治疗前记录根尖周疼痛水平和治疗后记录疼痛情况,采用卡方分析和二元logistic回归分析根管治疗术后疼痛的影响因素。结果:年龄(≥35岁)、性别(女性)、牙齿类型(磨牙)和术前中、重度疼痛在术后疼痛的发生率较高(P<0.05)。二元logistic回归分析显示,牙齿类型和术前根尖周疼痛是影响术后疼痛的危险因素(P<0.05);与非磨牙相比,磨牙术后疼痛的发生率相对较高(OR=3.23)。与术前无或轻度根尖周疼痛的患者相比,术前存在中、重度根尖周疼痛患者的术后疼痛发生率相对较高(OR=4.03)。结论:磨牙和术前存在中、重度根尖周疼痛是根管治疗术后疼痛的危险因素。
Objective To study the correlation between periapical periodontitis with preoperative periapical pain and postoperative pain.By considering the variables of sex,tooth type,jaw position and preoperative periapical pain,we measured their impact on the incidence of postoperative pain,so as to provide help for clinical prevention or relief of postoperative pain.Methods 436 consecutive patients who received root canal therapy in our department from January 2021 to January 2022 were selected for this study.After exclusion of screening criteria and other factors,133 cases(133 teeth)were included for further analysis.All patients recorded the demographic and sociological conditions and dental conditions.The patients were required to use visual analogue scale(VAS)to record periapical pain levels before and after treatment,and Chi-square analysis and binary logistic regression analysis were used.Results Age root canal therapy cases(≥35 years old),sex(female),tooth type(molar)and preoperative moderate and severe pain had a higher incidence of postoperative pain(P<0.05),Binary logistic regression analysis showed that tooth type and preoperative periapical pain were risk factors for postoperative pain(P<0.05).Compared with non molars,the incidence of postoperative pain in molars was relatively high(OR=3.23).For patients with no to mild periapical pain before surgery,the incidence of postoperative pain in patients with moderate to severe periapical pain before surgery was relatively high(OR=4.03).Conclusion Based on this study,it can be concluded that moderate to molar and severe periapical pain before root canal therapy are of postoperative pain.
