摘要
目的观察消痹散火疗风寒湿痹型膝骨性关节炎(KOA)患者的临床效果,并探讨其作用机制。方法选取2021年1月—2021年8月在福建中医药大学附属福州中医院骨科住院的60例风寒湿痹型KOA患者作为研究对象,根据随机数字表法随机分为对照组和干预组,各30例。对照组予福建中医药大学附属福州中医院骨科自制的常规中药热奄包治疗,干预组予消痹散火疗,观察2组美国特种外科医院(HSS)膝关节评分和白细胞介素1β(IL-1β)水平的变化。结果对照组、干预组总有效率分别为56.7%(17/30)和83.3%(25/30),2组间比较差异有统计学意义(P<0.05)。治疗前,2组患者HSS膝关节评分、血清IL-1β水平比较差异均无统计学意义(P>0.05);治疗后,干预组HSS膝关节评分明显高于对照组、血清IL-1β水平明显低于对照组(P<0.05)。结论消痹散火疗可明显缓解风寒湿痹型膝骨性关节炎患者关节疼痛等症状,降低其炎症反应,提高生活质量,是临床值得推广的中医外治法。
Objective To observe the clinical effect of fire therapy of Xiaobi powder in knee osteoarthritis of wind-cold-dampness type and explore the mechanism of action.Methods 60 patients with knee osteoarthritis of wind-cold-dampness hospitalized in Fuzhou Hospital of Traditional Chinese Medicine Affiliated to Fujian University of Traditional Chinese Medicine from January 2021 to August 2021 were selected as the research objects,and were randomly divided into control group and intervention group according to random number table method,with 30 cases in each group.The control group was treated with Chinese medicine fomentation package made in department of orthopedics of Fuzhou Hospital of Traditional Chinese Medicine Affiliated to Fujian University of Traditional Chinese Medicine,and the intervention group was treated with fire therapy of Xiaobi powder.The changes of knee HSS score and IL-1βof the two groups were observed.Results The total effective rate of the control group and the intervention group was 56.7%(17/30)and 83.3%(25/30),respectively,and the difference between the two groups was statistically significant(P<0.05).Before treatment,there were no significant differences in knee HSS score and serum IL-1βlevel between two groups(P>0.05).After treatment,knee HSS score and serum IL-1βlevel in intervention group were significantly higher than those in the control group(P<0.05).Conclusion Fire therapy of Xiaobi powder can obviously relieve the joint pain and other symptoms of patients with knee osteoarthritis of wind-cold-dampness type,reduce the inflammatory response and improve the quality of life.
