摘要
目的:探讨超声测量甲状腺容积在先天性甲状腺功能减退症(congenital hypothyroidism,CH)不同转归中的应用价值。方法:选取2009年1月—2019年1月在济宁市妇幼保健计划生育服务中心新生儿疾病筛查中心确诊的CH患儿743例作为研究对象,根据随访结果分为永久性甲减(permanent hypothyroidism,PCH)和暂时性甲减(temporary hypothyroidism,TCH),分析两组患儿的甲状腺超声情况。另选甲状腺功能正常的新生儿322名行甲状腺超声检查,获得济宁地区正常新生儿甲状腺容积参考值范围。结果:本地区322名正常新生儿甲状腺容积参考值范围为(0.790±0.256)m L。PCH与TCH患儿在甲状腺容积是否正常方面存在显著差异(P<0.01)。甲状腺容积与CH患儿的转归存在相关性(χ^(2)=120.107,P=0.001)。结论:超声测量甲状腺容积对CH患儿具有早期预测价值,既可帮助临床医师早期判断预后,还可减轻患儿家长的精神负担。
Objective To investigate the application value of ultrasonic thyroid volume measurement in different outcomes of congenital hypothyroidism(CH).Methods 743 cases children with CH diagnosed in the Neonatal Disease Screening Center of Jining Maternal and Child Health Family Planning Service Center from January 2009 to January 2019 were selected as subjects,permanent hypothyroidism(PCH)and temporary hypothyroidism(TCH)were divided according to the follow-up results,the thyroid ultrasound conditions of the two groups were analyzed statistically.322 newborns with normal thyroid function were selected for thyroid ultrasound examination to obtain the reference range of Jining area.Results The reference value of thyroid volume of normal neonates in this area was(0.790±0.256)mL.There is significant difference between PCH and TCH children in thyroid volume(P<0.01).There is a correlation between thyroid volume size and the outcome of children with CH(χ^(2)=120.107,P=0.001).Conclusion Ultrasound measurement of thyroid volume has early predictive value in children with CH,which can help clinicians to judge the prognosis in early stage and also reduce the mental burden of parents of children with CH.
